免疫酶染色试验在班氏丝虫病诊断中的应用

摘    要:应用马来丝虫成虫冰冻切片抗原作IEST。233例班氏丝虫患者IgG和IgM阳性率分别为94.0%和82.4%,其中微丝蚴血症者223例,IgG和IgM阳性率分别为94.2%和83.9%;该两种抗体总阳性符合率为96.0%。165名献血员IgG和IgM假阳性率分别为6.1%和1.2%。基本消灭丝虫病地区6~15与1~5岁年龄组的抗体水平无显著性差异,且当地未检出微丝蚴血症者,显示丝虫病传播基本阻断;而丝虫病流行区6~15岁年龄组抗体水平接近高峰,表明当地丝虫病仍在传播。提示以6~15岁年龄组人群特异性抗体水平变化与其他年龄组互相比较,可用于丝虫病流行病学监测。

Abstract:Immunoenzymatic staining technique (IEST ) using frozen sections of adult Brugia malayi as antigen was used to detect antibody in Bancroftian filariasis. Of 233 patients with Bancroftian filariasis, the positive rates of specific IgG and IgM antibody in the patients' ser- a were 94. 0% and 82. 4% respectively. In 223 cases with microfilaremia, the positive rates of IgG and IgM were 94. 2% and 83. 9% respectively. The total positive rate of IgG and/or IgM was 96. 0% and the false positive rates of IgG and IgM were 6. 1% (10/165) and 1. 2%(2/165) respec- tively. In an area,where Bancroftian filariasis had been basically eliminated,the antibody levels of both 6~15 age group and 1~5 age group were of no statistically significant difference and no mi- crofilaremia was detected, indicating the basic blockage of filariasis transmission in the locality. However,in Bancroftian filariasis endemic area the antibody level of 6~15 age group approximat- ed the peak among all the age groups, revealing that filariasis still continued to transmit. The studies suggest that the changes of specific antibody level in 6~15 age group, compared with those of other age groups,might be considered as an index in epidemiological surveillance of filari- asis.
