度洛西汀与文拉法辛缓释剂治疗抑郁症对照研究

目的探讨度洛西汀与文拉法辛缓释剂治疗抑郁症的临床疗效和安全性。方法将60例抑郁症患者随机分为两组，每组30例，研究组口服度洛西汀治疗，对照组口服文拉法辛缓释剂治疗，观察6周。于治疗前及治疗第2周、4周、6周末采用汉密顿抑郁量表评定临床疗效，副反应量表评定不良反应。结果治疗后两组汉密顿抑郁量表评分均较治疗前显著下降（P〈0．01），对照组治疗2周末较研究组下降更显著（P〈0．01），其他时段评分两组比较差异均无显著性（P〉0．05）；治疗6周末，研究组总有效率85．19％，对照组为92．00％，两组比较差异无显著性（χ^2=0．59，P〉0．05）。两组不良反应均轻微，主要表现为恶心、头昏、乏力、便秘等，多出现在治疗初期，随着治疗时间的延续可自行缓解或消失。结论度洛西汀与文拉法辛缓释剂治疗抑郁症疗效显著且相当，安全性高，依从性好，但文拉法辛缓释剂起效更快。

A control study of duloxetine vs. venlafaxine XR in the treatment of depression

Objective To explore the efficacy and safety of duloxetine and venlafaxine XR in the treatment of depression. Methods Sixty depression patients were randomly assigned to two groups of 30 ones each, research group took orally duloxetine and control group did venlafaxine XR for 6 weeks. Efficacies were assessed with the Hamilton Depression Scale （HAMD） and adverse reactions with the Treatment Emergent Symptom Scale （TESS） at baseline and at the end of the 2^nd, 4^th and 6^th week. Results The HAMD scores of both groups lowered more significantly after treatment compared with pretreatment （P〈0.01）, so did that in research than in control group at the end of the 2nd week （P〈0.01）, and there were no significant differences in other periods （P〈0.05） ; there was no significant difference in total effective rate at the end of the 6^th week （85.19% vs. 92.00% χ^2=0. 59,P）0.05）. Adverse reactions of both groups were mild and mainly nausea, dizziness, anergy, constipation and so on, most occurred in the initial stage of treatment, and remitted or disappeared spontaneously with treatment. Conclusion Both duloxetine and venlafaxine XR have an evident and equivalent effect in depression, higher safety, better compliance, but venlafaxine XR takes effect more rapidly.
