氯氮平与氯丙嗪对男性首发精神分裂症患者血糖的影响

摘    要:目的探讨氯氮平与氯丙嗪对男性首发精神分裂症患者空腹血糖的影响。方法将35例长期单一口服氯氮平治疗的男性首发精神分裂症患者设为氯氮平组,68例长期单一口服氯丙嗪治疗的男性首发精神分裂症患者设为氯丙嗪组,观察1a。于治疗前及治疗1a末检测两组空腹血糖。结果治疗1a末,两组空腹血糖水平均较治疗前显著升高(P〈0.05或0.01);氯氮平组空腹血糖水平高于正常值检出率为34.29%,氯丙嗪组为14.71%,氯氮平组显著高于氯丙嗪组(χ^2=5.27,P〈0.05)。结论氯氮平与氯丙嗪对精神分裂症患者的糖代谢均有显著影响,氯氮平影响更为显著,临床医生对长期服用氯氮平、氯丙嗪治疗的精神分裂症患者应定期检测血糖。

Abstract:Objective To explore the effects of clozapine and chlorpromazine on fasting blood sugar of male first-episode schizophrenics. Methods Thirty-five male first-episode schizophrenics on long-term single clozapine were assigned to clozapine group and 68 ones on long-term single chlorpromazine to chlorpromazine group, observed for 1 years. Fasting blood sugar was detected at baseline and at the end of 1 year. Results At the end of 1 year fasting blood sugar of both groups increased more significantly compared with pretreatment (P〈0.05 or 0.01) ; detection rate of elevated fasting blood sugar was respectively 34.29% in clozapine group and 14.71% in chlorpromazine group, the former was significantly higher than the latter (χ^2=5.27,P〈0.05). Conclusion Both clozapine and chlorpromazine have notable effects on glycometabolism of schizophrenics, clozapine's effect is more significant, so blood sugar should be regularly detec ted for schizophrenics who on long-term clozapine or chlorpromazine.
