产程中不同分娩体位对分娩效果的影响

目的探讨第一产程、第二产程中,不同的分娩体位对产妇分娩效果的影响。方法选取住院阴道分娩的足月妊娠单胎头位的初产妇360例,随机分为两组,即实验组第一产程采取自由体位,第二产程采用半卧位,适时截石位;对照组第一产程采取卧位,第二产程采用仰卧膀胱截石位。比较两组产程时间、分娩方式、阴道出血量、产妇舒适度及新生儿窒息发生率。结果实验组第一、第二产程时间、剖宫产率、产后2 h内出血量及新生儿窒息率显著低于对照组;实验组较对照组产后下肢疼痛、麻木不适减轻,舒适度增加。结论分娩过程中第一产程采用自由体位、第二产程半卧位适时截石位优于第一产程采取卧位、第二产程采取截石位。

Influence of different body positions on childbirth during birth process

Objective To study difference postures affecting the birth result during the first and the second birth process.Methods Chose 360 cases the primiparae who were pregnant 37 ~ 42 weeks and head posture delivery from vagina,and the parturients were divided into two groups by randomness.The parturients in experimental group took free positions in the first stage of labor,and half lying-position linking with appropriate vesical lithotomy positions were selcted in the second stage of labor;the parturients in control group took the decubitus（alternation between prostration and lateral position） during the first birth process,and routine lithotomy position of supine positions were selcted during the second birth process.Observed the parturients＇ birth process developing,complication and the changing of comfort in two groups.Results The first and second processes,cesarean section rate,postpartum bleeding in two hours,and new fetal suffocation in experimental group were shorter than that of the control group.Comparing with the parturients in control group,the parturients in experimental group reduced the frequence of lower limbs pain and increased comfort.Conclusion Tooking free positions in the first stage of labor,and half lying-position linking with appropriate vesical lithotomy positions in the second stage of labor was prior to selcting the decubitus during the first birth process,and routine lithotomy position of supine positions during the second birth process.
