肝硬化自发性腹膜炎患者血浆、腹水ICAM-1和趋化因子检测及其意义

目的评估血清和腹水中的IL-8和单核细胞趋化蛋白-1（MCP-1）、细胞间粘附分子（ICAM-1）的水平对自发性细菌性腹膜炎（SBP）的发病机制及预后的意义。方法用ELISA方法测定SBP组（n=18）、非感染性肝硬化腹水组（n=14）、健康对照组（n=15）血清和腹水的IL-8、MCP-1、ICAM-1的量。结果合并感染和无感染的肝硬化患者血清ICAM-1的水平较高。SBP组血清和腹水的IL-8和ICAM-1水平较高，同时腹水MCP-1浓度也较高；在SBP组死亡的患者中，ICAM-1的血清水平较高。结论腹水趋化因子的浓度增高预示SBP患者腹膜的炎症渗入，血清ICAM-1的水平是判断SBP预后的重要指标。

Examination of ICAM-1 and Chemokines in Serum and Ascites of Patients with Liver Cirrhosis Complicated with Spontaneous Bacterial Peritonitis

Objective To evaluate the significance of serum and ascitic fluid levels of IL-8, monocyte chemotactic protein-1 （ MCP-1 ）, and intercellular adhesion molecule-1 （ ICAM-1 ） in patients with spontaneous bacterial peritonitis （SBP）. Methods The cases were randomly divided into SBP group （n = 18 ）, non-infected cirrhotic control group （ n = 14） , and healthy control group （ n = 15 ）, and the levels of IL-8, MCP-1, and ICAM-1 in in serum and ascitic fluid were analyzed by ELISA. Results There was higher level of ICAM-1 in serum of Infected and non-infected cirrhotic patients than that in healthy control group. The levels of IL-8 and ICAM-1 in serum and ascitic fluid and MCP-1 in ascitic fluid were significantly elevated in SBP group compared with non-infected cirrhotic control group, and an elevated serum level of ICAM-1 was detected in dead patients of SBP group. Conclusion Elevation of chemokine levels in ascitic fluid could be implicated in the peritoneal infiltrate in patients with SBP. The level of serum ICAM-1 can be used as a evaluation index for prognosis of patients with SBP.
