普通人工股骨头置换治疗高龄股骨转子间不稳定性骨折

目的回顾性评估应用普通双动人工股骨头治疗高龄不稳定股骨转子间骨折的疗效。方法 2005-2009年使用普通人工股骨头置换术治疗高龄不稳定股骨转子间骨折患者12例,EvansⅢ型7例,Ⅳ型5例,平均年龄82.7岁(78~89),术后随访8~32个月,平均19.4个月。Harris评分评价关节功能、活动能力。术后2周内、3~6个月、1年拍患髋X线片,依据Cruen分区检查假体有无松动迹象。结果手术时间平均90m i,出血量平均300 m l,10例术后5~10 d下床负重行走,2例3周后开始负重。Harris评分:优4例,良6例,中2例,优良率83.3%。12例术后X线片检查未见假体松动现象。结论应用双动骨水泥型人工股骨头置换对高龄不稳定性股骨转子间骨折患者是一种有效的治疗方法。

Treatment of Unstable Femoral Intertrochanteric Fractures In Elderly Patients With Artificial Femoral Head Arthyroplasty

Objective To evaluate the effect of prosthetic replacement to treat unstable femoral intertrochanteric fractures in elderly patients.Methods From 2005 to 2009,12 cases were treated.According to the classification of Evans,there were 7 patients with type III,5 were with type IV.The average age of patients was 82.7 years（78-89years）.All patients were followed up for 8-32 months（19.4 months averagely）.Results The mean duration of operation was 90 minutes and blood loss was 300ml.Weight-bearing walking began at 5 to 10 days after the operation.According to Harris hip score standard features： excellent recover was observed in 4 cases,good in 6 cases,moderate in 2 cases,cases,and the excellent/good rate was 83.3%.No infection and loosening were found.Conclusions It is an effective to treat unstable femoral intertrochanteric fracture with prosthetic replacement in elderly patients.
