^131碘全身显像、甲状腺球蛋白测定及颈部超声检查在分化型甲状腺癌^131碘治疗随访中的意义

目的：探讨^131I一全身显像（^131I—WBS）、甲状腺球蛋白（Tg）测定及颈部B超检查在分化型甲状腺癌术后^131I治疗随访中的意义。方法：31例分化型甲状腺癌术后^131I治疗患者，治疗前后及随访中行甲状腺吸^131I率、颈部B超、^131I显像及甲状腺球蛋白测定。结果：治疗剂量^131I—WBS较诊断剂量^131I—WBS多发现3例转移灶；64人次Tg与^131I—WBS检查结果：Tg与^131I—WBS均阳性者19／64，均阴性者35／64，Tg阳性而^131I—WBS阴性者5／64，Tg阴性而^131I—WBS阳性者5／64；B超发现颈部淋巴结增大者为80．6％。结论：^131I治疗分化型甲状腺癌转移灶效果突出，^131I—WBS、Tg测定与颈部B超在其随访中联合应用可以相互补充，具有重要意义。

The value of 131I -whole body scan,thyroglobulin level and cervical ultrasonic examina- tion in the follow -up of differentiated thyroid carcinoma with 131I radiotherapy

Objective： To investigate the value of 1311 - whole body scan （ 131 1 - WBS）, the serum thyroglobulin （Tg） and cervical ultrasonic examination on the follow- up of differentiated thyroid carcinoma （DTC） with 131I ra- diotherapy after surgical treatment. Methods：Thirty one patients with DTC after surgical treatment were treated with 131I radiotherapy. 131I --WBS, serum Tg levels and cervical ultrasonic examination were performed before and after 131I radiotherapy. Results ：Three additional cases were detected in therapeutic dosage of I31I - WBS than in diagnostic dos- age of 131I -WBS, 19 cases were found positive with 131I --WBS and Tg level examination while 35 cases were proved negative ,5 cases were positive with 131I -WBS but Tg levels were normal. On the other hand,5 cases with normal 131I -WBS had increased Tg levels, 80.6% enlarged cervical lymph nodes were detected by cervical ultrasonic measurement. Conclusion：131I radiotherapy has an excellent therapeutic results, 131I -WBS, Tg and cervical ultrasonic exami- nation combined measurement has a satisfied clinical value on the follow - up of DTC with 131 I therapy after surgical treatment.
