三维超声体积自动测量技术测量先心病胎儿肺体积准确性的研究

摘    要:目的探讨三维超声体积自动测量技术(VOCAL)测量先心病胎儿肺体积的准确性和可靠性。方法以21例先心病胎儿为研究对象,对比分析引产前后三维超声VOCAL技术检测胎儿肺体积与尸检排水法测值的变化情况。结果(1)21例先心病胎儿孕妇,年龄19~35岁,孕周23.1~27.0周。(2)三维超声VOCAL法与尸检排水法测量胎儿肺体积的均值分别为(20.79±4.70)、(20.04±4.87)ml,两者体积均值的差异无统计学意义(P>0.05)。(3)三维超声VOCAL技术与胎儿尸检排水法实际肺体积测值之间的差值为(-0.75±1.72)ml,其95%可信区间为-4.12~2.62ml。Bland-Altman分析表明三维超声VOCAL法与尸检排水法测量胎儿肺体积具有较好的一致性。结论三维超声VOCAL技术测量先心病胎儿肺体积准确性高,具有无创性,这为产前准确评估先心病胎儿肺体积发育提供了可靠的测量方法。

Abstract:Objective The objective of this study was to evaluate the accuracy of three-dimensional ultrasound virtual organ computer-aided analysis(VOCAL)technique in measurement of fetal lung volume.Methods Fetal lung volume was measured during 3 Dultrasound examination using three-dimensional ultrasound VOCAL technique in 21 cases of congenital heart disease(CHD),immediately before termination.The 3 Dultrasound results were compared with postmortem measurement of fetal lung volume achieved by water displacement.Results 21 fetuses with CHD were selected for the study.The maternal age were 19-35 years,and the gestational ages were 23.1-27.0 weeks.The volume of the 21 fetuses lung was(20.79±4.70)ml with 3 D-ultrasound(VOCAL),and(20.04±4.87)ml with water displacement method.And there was no significant difference between the 3 D-ultrasound results and the actual volume(P>0.05).The difference of the two methods was(-0.75±1.72)ml,and the 95%limits of agreement was-4.12-2.62 ml.Bland-Altman analysis shows that two methods of measuring fetal lung volume have good consistency.Conclusions The 3 D-ultrasound could measure the fetal lung volume accurately.
