胰腺神经内分泌肿瘤的超声诊断分析

目的探讨彩色多普勒超声对胰腺神经内分泌肿瘤(pNETs)的诊断价值。方法回顾性分析31例经病理证实的pNETs的彩色多普勒超声表现,并与临床资料及术后病理进行对照分析。结果 28例超声检出的pNETs声像图表现病灶内部多为实性结构(24/28),多呈低回声(24/28),回声多较均匀(19/28),边界多较清晰(19/28),形态较规则(20/28),血供多为0~Ⅰ级(22/28)。不同病理分级pNETs之间声像图比较,G1级与G3级两组之间仅在边界、形态方面差异有统计学意义(P=0.021、0.039);G1级与G2级两组之间仅在边界方面差异有统计学意义(P=0.035);G2级与G3级两组之间仅在形态方面差异有统计学意义(P=0.035)。结论 pNETs的声像图表现具有一定的特征性,综合患者的临床症状及肿瘤标志物检查,将有助于超声对pNETs的诊断与鉴别诊断。

Analysis of Ultrasonographic Diagnosis of Pancreatic Neuroendocrine Tumors

Objective To explore the value of color doppler ultrasound in diagnosis of pancreatic neuroendocrine tumors(pNETs).Methods The ultrasonic manifestations of 31 patients with pNETs were analyzed and compared with clinical features as well as postoperative pathological findings retrospectively.Results The sonographic features of pNETs detected by ultrasound in 28 patients included solid internal structures(24/28),hypoechogenicity(24/28),uniform echogenicity(19/28),well-defined boundary(19/28),regular shape(20/28)and grade 0-Ⅰ blood flow pattern(22/28).Compared with the sonographic images of pNETs with different pathological grades,the difference between G1 and G3 grades was only statistically significant in the boundary and morphology(P=0.021,0.039).The difference between G1 and G2 grades was statistically significant only in the boundary between the two groups(P=0.035).The difference between G2 and G3 grades was only statistically significant in morphology(P=0.035).Conclusions There are some specific sonographic features of pNETs.A comprehensive analysis of the patient′s clinical symptoms and tumor markers will be helpful for ultrasonic diagnosis and differential diagnosis of pNETs.
