超声造影检查鉴别诊断门静脉栓子良恶性的价值分析

摘    要:目的探讨超声造影诊断门静脉栓子良恶的价值。方法分析经临床证实的12例门静脉血栓和26例门静脉癌栓的超声造影技术(CEUS)以及彩色多普勒血流成像技术(CDFI)特征,比较造影后栓子内血流灌注特征。结果门静脉癌栓26例,CDFI诊断18例(69.2%),CEUS诊断26例(100%),癌栓造影呈"快进快出"特点;门静脉血栓12例,均无增强。结论超声造影对鉴别门静脉栓子良恶性有较高价值,值得临床推广。

Abstract:Objective To explore the value of contrast-enhanced ultrasound in the diagnosis of portal vein emboli.Methods The soreqraphic featares of contrast-erhamced ultrasonograply(CEUS)and color doppler flow image(CDFI)were analyzed in 12 cases with portal venous thrombosis and 26 cases with portal venous carcinoma thrombus who were confiraed clinically.The characteristic of blood flow perfusion in embolus were compared after CEUS.Results There were 26 cases of portal venous carcinoma thrombi,18 cases(69.2%)were diagnosed by CDFI,and 26 cases(100%)were diagnosed by CEUS.The CEUS of tumor thrombus showed the characteristic of "fast in and fast out".Portal vein thrombosis was found in 12 cases without enhancement.Conclusions Contrast-enhanced ultrasonography has a high value in differentiating benign and malignant portal vein emboli and is worthy of clinical promotion.
