参芪补肺汤治疗肺气虚型慢性阻塞性肺疾病稳定期临床疗效的荟萃分析

目的 荟萃分析参芪补肺汤运用到肺气虚型慢性阻塞性肺疾病稳定期的临床疗效和安全性。方法 通过运用综合检索策略在PubMed,Cochrane,CNKI,中国生物医学管理系统,VIP,万方等数据库中共检索到了99篇文献,通过严格的纳入排除标准剔除各种混杂文献最终剩余10篇RCTs,包含966个样本量,荟萃分析软件使用RevMan 5.3进行。结果 系统评价结果可见：肺气虚型慢性阻塞性肺疾病稳定期患者的临床总有效率经参芪补肺汤治疗后得到显著提高,在总有效率中参芪补肺汤组均大于对照组［Z = 6.79,95%CI范围（1.16,1.31）,P < 0.05］。此外在部分肺功能指标FVC,FEV1,FEV1/FVC参芪补肺汤组也均优于对照组：［MD = 0.23,95%CI范围（0.13,0.57）,P < 0.05］,［MD = 0.11,95%CI范围（0.06,0.15）,P < 0.05］,［MD = 9.36,95%CI范围（5.13,13.59）,P < 0.05］;同时症状评分及６分钟行走距离参芪补肺汤联合常规治疗多优于单纯西医常规治疗;不良反应的发生参芪补肺汤组也低于对照组［Z = 2.15,95%CI（0.17,0.92）,P = 0.03］。结论 参芪补肺汤联合西医常规治疗对于总有效率、肺功能指标、症状评分、6-minute walk distance和减少不良反应的发生等方面多优于单纯的西医常规治疗。

A Meta-analysis of the Clinical Efficacy of Shenqi Bufei Decoction in the Treatment of Chronic Obstructive Pulmonary Disease with Pulmonary Qi Deficiency at Stable Stage

Objective To meta-analyze the clinical efficacy and safety of Shenqi Bufei Decoction in chronic obstructive pulmonary disease (COPD) with pulmonary qi deficiency. Methods A total of 99 literatures were retrieved in PubMed, Cochrane, CNKI, China biomedical management system, VIP, Wanfang and other databases by using comprehensive retrieval strategies. The final remaining 10 RCTs were excluded by strict inclusion and exclusion criteria, including 966 samples. RevMan 5.3 was used for meta-analysis. Results The results of the systematic evaluation showed that the total clinical effective rate of patients with chronic obstructive pulmonary disease of lung-qi deficiency type was significantly improved after the treatment with Shenqi Bufei Decoction. In the total effective rate, Shenqi Bufei Decoction group was larger than the control group Z = 6.79, 95%CI (1.16, 1.31), P < 0.05]. In addition, partial lung function indicators FVC, FEV1, FEV1/FVC Shenqi Bufei Decoction group were also better than the control group: MD = 0.23, 95%CI (0.13, 0.57), P < 0.05], MD = 0.11, 95%CI (0.06, 0.15), P < 0.05], MD = 9.36, 95%CI (5.13, 13.59), P < 0.05]. At the same time, the symptom score and 6-minute walking distance of Shenqi Bufei Decoction combined with conventional treatment were more superior to the conventional treatment of western medicine alone. The occurrence of adverse reactions in the Shenqi Bufei Decoction group was also lower than that in the control group Z = 2.15, 95%CI (0.17, 0.92), P = 0.03]. Conclusion Shenqi Bufei Decoction combined with conventional western medicine is better than conventional western medicine in total effective rate, lung function index, symptom score, 6-minute walk distance and reducing the occurrence of adverse reactions.
