儿童横纹肌肉瘤17例病例分析

目的探讨儿童横纹肌肉瘤(RMS)的规范诊断与治疗。方法回顾性分析17例儿童RMS的临床资料,结合美国国际横纹肌肉瘤研究组(IRSG)的研究,总结在诊断及治疗上与国外的异同及存在的临床问题。结果 11例患儿未作活检确诊而直接手术。所有手术患儿均无手术切缘病理。2例诊断后放弃治疗。治疗的15例患儿均行化疗,疗程6~25个不等;仅6例行放疗;12例结束治疗的患儿中7例复发。结论本组病例表明,诊疗欠规范是导致临床结局较差的重要原因。因此,规范、准确的诊断及治疗是提高儿童RMS疗效、减少复发的有效途径。

Analysis on diagnosis and treatment of 17 cases of children rhabdomyosarcoma

Objective To explore the standardized diagnosis and treatment of rhabdomyosarcoma (RMS).Methods Clinical datas of seventeen children patients with RMS were retrospectively reviewed.Based on the study of IRSG,we summarized the similarities and differences between our work and abroad,and discussed the clinical problems as well.Results Eleven cases undergone surgery other than initial biopsy to determine the diagnosis.None of the patient had surgical cutting edge pathology.Two cases gave up treatment after diagnosis.Fifteen cases received combined chemotherapy,and only six of them received additional local radiotherapy.Seven out of 12 were relapsed after finishing the whole treatment.Conclusions The lack of standardized diagnosis and treatment of RMS is a major reason for the poor outcomes.Standardized diagnosis and treatment of RMS is a good way to improve therapeutic efficacy and long-term outcomes.
