CA19-9和肝功能联合检测用于鉴别诊断胰腺癌和胰腺炎

探讨CA19-9及肝功能各生化指标水平在胰腺癌、急慢性胰腺炎患者血清中的变化规律,探索多指标联合检测用于鉴别诊断胰腺癌和胰腺炎的临床价值。采用电化学发光免疫法与生化比色法分别检测70例胰腺癌患者、43例急性胰腺炎患者、25例慢性胰腺炎患者及65名正常人血清中CA19-9和肝功能各生化指标的含量。结果显示,与对照组相比,胰腺癌组、急慢性胰腺炎组的ALB水平均显著降低（P〈0.01）,CA19-9、TBA、DBIL、AST、γ-GT及ALP水平均显著升高（P〈0.01）,TBIL及ALT水平的差异无显著意义（P〉0.05）。胰腺癌组与急性胰腺炎组相比,CA19-9及ALB水平显著升高（P〈0.01）,γ-GT水平显著降低（P〈0.05）;与慢性胰腺炎组相比,仅有CA19-9水平显著升高（P〈0.01）。两胰腺炎组比较,各指标的差异均无显著意义（P〉0.05）。胰腺癌组内,CA19-9与肝功能多个指标有相关性,但与γ-GT的相关性较显著（r=0.339,P=0.004）;慢性胰腺炎组内CA19-9仅与γ-GT有显著的正相关（r=0.570,P=0.006）;急性胰腺炎组内,CA19-9与TBA、DBIL、ALT、AST、γ-GT及ALP均有显著的正相关（r〉0.5,P〈0.01）。胰腺癌、急慢性胰腺炎患者存在不同程度的肝功能损害,因此,同时测定CA19-9和肝功能各生化指标对胰腺癌和急慢性胰腺炎具有一定的辅助诊断价值。
