正常人及急性心肌梗死患者心率变异频谱分析

摘    要:目的:运用心率变异(HRV)频谱分析法研究正常人和急性心肌梗死(AMI)患者心脏植物神经功能变化。方法:114例正常人和27例AMI患者先记录24h动态心电图,将全部窦性心率经计算机HRV软件处理后使心电信号转换为心率功率频谱(HRPS)。结果:正常人HRPS的低频段功率密度(PSD)、高频段功率密度在白天和夜间有明显的差异性(P<0.01),白天低频段PSD明显增加,夜间高频段PSD明显增加;随着年龄的增加,低频段PSD和高频段PSD逐渐下降。AMI患者低频段PSD和高频段PSD与正常人比较明显降低(P<0.01),且无白昼生理波动,这说明AMI患者支配心脏的交感神经和迷走神经均受损。结论:HRV频谱分析对评价心脏植物神经功能是一种敏感而无创的检测指标。

Abstract:Aim: To analyse the functional variation of cardiac autonomic nerve in normal persons and patients suffering from acute myocardial infarction (AMI). Methods: 24 hours dynamic ECG of 114 normal persons and 27 patients of AMI were recorded firstly,then all sinus rhythms were changed into the heart rate power spectrum with HRV software in the computer. Results: It is demonstrated that low frequency power spectral density (PSD) and high frequency PSD fluctuate in normal persons (P<0. 01); low frequency PSD increases in the daytime, but high frequency PSD rises at night; low frequency PSD and high frequency PSD both reduced with the raising of age, especially low frequency PSD reduced markedly. In the patients of AMI, all parameter mentioned above reduced markedly and the fluctuation of these parameter decreased at all the time of a day. These results showed that the function of sympathetic and vagal nerve of heart were injured markedly in the patients of AMI. Conclusion: It is suggested that the analysis of power spectrum of HRV be a sensitive noninvasive method to evaluate the function of cardiac autonomic nerve.
