经皮腘静脉穿刺诊治下肢深静脉血栓39例临床分析

目的探讨经皮腘静脉血流顺向性介入治疗下肢深静脉血栓的可行性。方法对39例下肢深静脉血栓患者经健侧股静脉置入下腔静脉滤器后,穿刺患侧腘静脉,以腘静脉为入路,行血管内溶栓、血栓消融器消融和(或)球囊成形及腔内支架置人术等治疗。结果经皮腘静脉穿刺均获成功,无严重并发症发生。结论以患侧腘静脉为入路,血流顺行性介入治疗下肢深静脉血栓操作简便,是一种安全有效的方法。

Percutaneous popliteal vein puncture for diagnosis and treatment of lower extremity deep venous thrombosis

Objective To evaluate the feasibility of percutaneous popliteal vein blood antegrade interventional treatment for lower extremity deep venous thrombosis (LEDVT).Methods Inferior vena cava filters(IVCF) were placed in 39 cases of LEDVT via the normal femoral veins.Puncture was carried out through the popliteal veins in the thrombotic side.Therapies were accomplished via the popliteal vein,including intravascular thrombolysis,thrombosis amplification with Amplatz Thrombectomy Device (ATD),angioplasty, and/or stent implantation.Results Puncture of percutaneous popliteal vein in all cases was accomplished successfully and no serious complication occurred.Conclusion Blood antegrade intervention via the popliteal vein in the thrombotic side is a safe and effective therapy for lower extremity deep venous thrombosis.
