Modified exotropia surgery for intermittent exotropia in children with low dioptric convergence insufficiency type

Objective To study the effect of 2.5~3.0 mm unilateral lateral muscle recession combined with medial rectus muscle slanted resection in treating intermittent exotropia of children with slow dioptric convergence insufficiency type.Methods A retrospective study was carried on 31 patient with small-diopter convergence insufficiency type exodeviation of 20^△ to 30^△ at near,mean distance angle of deviation preoperatively and postoperatively;mean near angle of deviation preoperatively and postoperatively;and mean near-distance exodeviation differences preoperatively and postoperatively;besides,positioning rates,A-V pattern,eye movement,stereoscopic visions,preoperatively and postoperatively.The follow-up period ranged from 6 to 12 months.Results Preoperatively,the average angle of deviation of near and distance exodeviation was 26.4±1.2^△ and 13.2±2.1^△,near-distance exodeviation differences 18.3±2.4^△ the average stereoacuity was 461.2 "postoperatively,the average angle of deviation of near and distance exodeviation was 6.4±1.1^△(t=8.72,P<0.05) and 3.2±0.8^△(t=3.46,P <0.05),near-distance exodeviation differences 4.3±1.3^△(t=5.75,P<0.05), the average stereoacuity was 361.2"(t=3.25,P<0.05),positioning rates 83.9%(26/31),there was no A-V pattern and limitation of eye movement.Conclusion 2.5~3.0 mm unilateral lateral muscle recession combined with medial rectus muscle slanted resection was safe and effective in treating intermittent exotropia of children with slow dioptric convergence insufficiency type,reducing angle of near and distance,and difference of near-diatance angle,improving orthophoria rate,restoring stereoscopic vision.
