急性心肌梗死中医辨证分型的聚类研究

目的观察急性心肌梗死(AMI)患者中医辨证分型的特点及规律。方法利用冠心病个体化诊疗研究平台,采用SPSS12.0统计软件包,对744例AMI患者的四诊信息进行K-means聚类分析,再对每种类型进行主成分分析,判定每种类型的代表症状,归纳出证型。结果调查收集AMI患者的症状共有46种,症状频率出现较高的为胸闷、胸痛、乏力、气短、心悸、喘憋;舌质多见暗红、淡暗、紫暗,舌苔以腻、白为主;常见的脉象为弦滑、弦、滑、弦细;总结出八种证候群。结论根据聚类分析结果并结合临床实践,认为AMI患者分为八型较符合临床实际,聚类分析有助于中医辨证客观化研究。

The Cluster Analysis on Synorome Type of TCM in Patients with Acute Myocardial Infarction

Objective: To explore the characteristics of TCM manifestation in patients with acute myocardial infarction and the distribution rules of Syndrome Type of TCM.Methods: The patients' information was collected by system of individualized diagnostic and treatment for coronary heart disease. These data were analyzed by SPSS 12.0 software. K-means cluster analysis was used to classify the information of four physical examinations of 744 cases of acute myocardial infarction. Every Syndrome Type of TCM was analyzed by principal component analysis and the representative symptoms were determined. The Syndrome Type of TCM in patients with acute myocardial infarction was concluded.Results: There were 46 symptoms in patients with acute myocardial infarction. Chest distress,chest pain,debility,short breath,palpitation and dyspnea were present frequently. The most tongue body were dark-red tongue,plae-dark tongue and purple-dark tongue,and the principal tongue coating was greasy fur and white fur. The mainly pulse conditions were wiry-slippery pulse,wiry pulse,slippery pulse and wiry-Concthready pulse. Eight Syndrome Types of TCM were concluded.Conclusion: Integrating the results of cluster analysis with clinic practice,we conclud that the Syndrome Types of TCM in patients with acute myocardial could be summed up into eight TCM syndrome types. Cluster analysis is contributes to objective research of Syndrome Type of TCM.
