男性垂体泌乳素瘤经鼻蝶手术治疗对血清睾酮的影响

目的:研究男性垂体泌乳素瘤患者经鼻蝶手术对血清睾酮的影响。方法:回顾性分析显微手术治疗的男性垂体泌乳素瘤15例,对手术前后血清泌乳素(PRL)和睾酮(T)进行测定及性功能随访。结果:11例手术全切肿瘤患者的术后血清RRL水平较术前明显下降;血清T在术后1个月明显上升。10例性功能障碍患者有6例改善。但是另外4例由于肿瘤呈侵袭性生长或特别巨大,无法手术全切,术后辅助药物治疗,血清T水平在术后6个月仍无法达到正常水平。结论:经蝶显微手术切除肿瘤或手术辅助药物治疗,有助于提高术后血清T水平。术后PRL水平、肿瘤的生长方式和手术切除程度是影响此类患者术后性功能恢复的主要因素。

Effects of Transsphenoidal Microsurgery on Serum Testosterone of Male Patients with Prolactinoma

Objective: To assess the surgical influence on serum testosterone of male patients with prolactinoma. Methods: Micro-surgical procedure was performed on 15 male patients with prolactinoma, and the surgical therapeutic effects and changes of endocrine functions were retrospectively analyzed. Results: After surgery, in total removal cases, the serum prolactin of them decreased significantly, and serum testosterone went back to normal 1 month after operation. Six of 10 cases improved their sexual function. But in other 4 cases, because tumors were invasive or very large, they can not be totally resected. Serum testosterone in these cases couldn't go back to normal after 6 months and auxiliary medicine treatment was needed to reduce serum prolactin. Conclusion: Micro-surgical tumor removel or surgery with medicine treatment can be helpful to improve serum testosterone in male cases. The post-surgery serum prolactin, the growth pattern of tumor, the extent of tumor removal are three main influencing factors of recovery of sexual function in male cases.
