阻塞性睡眠呼吸暂停综合征对高血压患者血压的影响

目的：观察阻塞性睡眠呼吸暂停综合征（OSAS）对高血压患者24h动态血压及血压昼夜节律的影响。方法：102例高血压患者根据多导睡眠仪监测结果分为单纯高血压组（53例），高血压合并轻度OSAS组（23例），高血压合并中重度OSAS组（26例），观察各组的动态血压及血压昼夜节律。结果：高血压合并中重度OSAS组、高血压合并轻度OSAS组与单纯高血压组比较，夜间平均舒张压、夜间平均收缩压升高，血压昼夜节律下降，差异有统计学意义（P〈0．05）。结论：阻塞性睡眠呼吸暂停加剧了高血压患者夜间血压的升高及血压昼夜节律异常，应重视对高血压合并阻塞性睡眠呼吸暂停综合征患者，行24h动态血压监测。

The effect of obstructive sleep apnea syndrome on blood pressure in patients with hypertension

Objective To study the effect of obstructive sleep apnea syndrome(OSAS) on blood pressure and the diurnal rhythm in patients with hypertension.MethodBased on the results of polysomnography(PSG),102 patients were divided into pure hypertension group(n=53),hypertension combined with mild OSAS group(n=23) and hypertension combined with moderate-severe OSAS group(n=26).They all received 24-hour blood pressure monitoring.ResultsCompared with pure hypertension group,there were significant differences in nocturnal SBP and DBP in hypertension with mild OSAS group and hypertension with moderate-severe OSAS group(P<0.05).There was significant difference in the proportion of non-dipper between pure hypertension group and hypertension with mild OSAS group(P<0.05) and was more significant in the proportion of non-dipper between pure hypertension and hypertension with moderate-severe OSAS group(P<0.01).ConclusionOSAS aggravates the blood pressure rising under sleep condition and abnormal diurnal rhythm of blood pressure of hypertensive patients.More attention shall be paid to monitor the 24-hour blood pressure for the hypertensive patients who combined with OSAS.
