超微血管成像背景下超声造影在甲状腺乳头状癌患者颈侧区淋巴结中的鉴别诊断价值

目的探讨超微血管成像(SMI)技术背景下超声造影(CEUS)对甲状腺乳头状癌(PTC)患者颈侧区淋巴结良恶性的鉴别诊断价值。方法选取在郑州大学第一附属医院行PTC手术及颈部淋巴结清扫术的63例患者,共计98枚淋巴结,术前均行彩色多普勒(CDFI)、超微血管成像(SMI)、SMI+超声造影(CEUS),观察淋巴结的血管模式,并进行统计学分析。结果SMI+CEUS对淋巴结良恶性的诊断价值高于单纯使用SMI(Z=2.586,P=0.010)、CDFI(Z=3.512,P<0.001),SMI对淋巴结良恶性的诊断价值优于CDFI(Z=2.210,P=0.027)。结论SMI对PTC患者颈侧区淋巴结转移具有较高的诊断效能,SMI联合CEUS诊断效能更高。

Clinical diagnostic value in the neck lateral lymph node of papillary thyroid carcinoma by contrast-enhanced ultrasound in the context of superb microvascular imaging

Objective To explore the diagnostic value of contrast-enhanced ultrasound(CEUS)in the context of superb microvascular imaging(SMI)in diagnosis of neck lateral lymph node of patients with papillary thyroid carcinoma.Methods A total of 98 lymph nodes were selected from 63 patients who underwent PTC surgery and cervical lymph node dissection in our hospital.Color doppler ultrasound(CDFI),superb microvascular imaging(SMI),SMI+contrast-enhanced ultrasound(CEUS)were performed before surgery to observe the lymph node blood flow pattern and conduct statistical analysis.Results The diagnostic value of SMI+CEUS in the lymph node benign and malignant was higher than those of SMI(Z=2.586,P=0.010)and CDFI(Z=3.512,P<0.001)alone,the diagnostic value of SMI for benign and malignant lymph nodes was superior to CDFI(Z=2.210,P=0.027).Conclusion SMI has a high diagnostic efficiency for preoperative cervical lymph node metastasis of thyroid papillary carcinoma,and the combination of SMI and CEUS can improve the diagnostic accuracy of cervical lymph nodes.
