2020年周口骨科医院危急值统计分析

目的统计我院2020年度医学检验科危急值数据及分布情况,为临床危急值项目及界限的设定提供参考,并优化危急值的临床应用。方法利用Microsoft Excel 2016软件按科室与检验项目统计分析我院检验科2020年危急值的登记记录数据。结果 2020年共报告危急值1 704例,占全年标本比例为0.68%,其中危急值数量较多的科室是重症医学科(24.9%)、内三科(23.9%)、脊柱中心(8.5%);危急值数量占比较高的检验项目为N端脑钠肽(24.5%)、D-二聚体(17.3%)、降钙素原(16.8%)、超敏肌钙蛋白(10.0%)和纤维蛋白原(9.7%);有38.7%、21.1%的降钙素原和D-二聚体危急值分布在重症医学科,有49.3%、55.9%的N端脑钠肽、超敏肌钙蛋白危急值分布在内三科。结论医院应定期统计分析医学检验科危急值数据,评估医学检验科危急值项目及界限在每个临床科室的适宜性及阈值范围的准确性,不断改进,以便提高医学检验科与临床科室的工作效率,更好地服务于临床诊疗和患者。

Statistical analysis of critical value of Zhoukou orthopedic hospital in 2020

Objective To statistics the data and distribution of critical value in medical laboratory of our hospital in 2020,and provide reference for setting the items and limits of clinical critical value and optimize the clinical application of critical value.Methods The registration data of critical value in our hospital in 2020 were counted and analyzed according to department and test items by Microsoft Excel 2016 software.Results In 2020,a total of 1704 critical values were reported,accounting for 0.68%of the samples in the whole year;the departments with more critical value were critical care department(24.9%),third internal medicine department(23.9%)and spinal center(8.5%);the test items with higher proportion of critical value quantity were N-terminal brain natriuretic peptide(24.5%),D-dimer(17.3%),procalcitonin(16.8%),hypersensitive troponin(10.0%)and fibrinogen(9.7%);38.7%of procalcitonin critical values and 21.1%of D-dimer critical values were distributed in critical care department,and 49.3%of N-terminal brain natriuretic peptide critical values and 55.9%of hypersensitive troponin critical values were distributed in the third internal medicine department.Conclusion The hospital should regularly statistics and analyze the critical value data of the medical laboratory,and evaluate the suitability and the accuracy of the threshold range of the critical value items and limits of the medical laboratory in each clinical department,and constantly improve,so as to improve the work efficiency of the medical laboratory and clinical departments,and better serve the clinical diagnosis and treatment and patients.
