髁间窝退行性变的诊断与处理

目的探讨股骨髁间窝退行性变的诊断及处理。方法本组共70膝，男15膝，女55膝，年龄31～74岁，平均60．4岁。患者主要表现为膝关节前部或定位不确定的钝痛、渐进性关节伸屈受限和反复关节积液。在关节镜下本病表现为：骨与骨撞击和骨与韧带、软组织的撞击两类。治疗采用关节镜下股骨髁间窝扩大成形术加镜下关节清理术。结果随访48例50膝，平均39个月，术后关节功能均超过术前水平，尤其关节伸屈功能改善明显。结论通过关节镜手术可有效完成对本病的诊断与治疗，关节镜下股骨髁间窝扩大成形术是治疗本病的好方法。

Arthroscopic Diagnosis and Management of Degenerative Lesion of Femoral Intercondylar Fossa

Objective To investigate arthroscopic diagnosis and management of degenerative lesion of femoral intercondylar fossa.Methods 70 knees(male 15,female 55) suffered from osteoarthritis with mean age 60.4 years(ranged from 31 to 74 years).The main symptoms were pain,restriction of knee movement and swelling recurrently.Two types of distinct arthroscopic appearances were noted: the impingement of bone to bone and bone to ligament or soft tissue.The treatment was all performed by arthroscopic arthroplasty. Results Forty-eight patients(50 knees) were followed-up for an average of 39 months.Good function was seen in all patients(especially in extension and flexion).Conclusion The authors emphasize that arthroscopy and arthroscopic arthroplasty were helpful to the establishment of the diagnosis and useful for treatment.
