翼状胬肉逆行切除联合自体结膜或角膜缘干细胞移植术的疗效观察

目的 观察翼状胬肉逆行切除联合自体结膜或角膜缘干细胞移植术防止翼状胬肉复发的疗效.方法 将73例(82只眼)翼状胬肉患者分为结膜组36例(41只眼)和干细胞组37例(41只眼),两组分别行翼状胬肉逆行切除联合自体结膜移植术和翼状胬肉逆行切除联合角膜缘干细胞移植术.观察术后两组角膜上皮修复时间和胬肉复发情况.结果 术后随访2年,两组患者均无睑球粘连和眼球活动受限等并发症.干细胞组角膜上皮平均修复时间为(5.7±1.3)d,胬肉复发率为4.9%(2/41),结膜组角膜上皮平均修复时间为(6.1±1.1)d,复发率为9.8%(4/41),两组比较差异均无统计学意义(P＞0.05).结论 翼状胬肉逆行切除联合自体结膜或角膜缘干细胞移植术均能有效降低胬肉的复发率.(中国眼耳鼻喉科杂志,2010,10:303-305)

Observation of the efficacy of retrograde pterygium excision combined with autologous conjunctival or corneal limbal stem cell transplantation

Objective To observe the efficacy of retrograde pterygium excision combined with autologous conjunctival or corneal limbal stem cell transplantation on the treatment of pterygium. Methods Seventy-three patients （82 eyes） of pterygium were randomly divided into conjunetiva] group of 36 patients （41eyes） and stem cell group of 37 cases （41eyes）. The two groups were treated by retrograde pterygium excision combined with autologous eonjunctival transplantation surgery and retrograde pterygium excision combined with limbal stem cell transplantation respectively. Corneal epithelial repair time and pterygium recurrence was observed after the surgery in both groups. Results Patients of the two groups received a 2 years＇ fullow-up, no eomplications such a.s symblepharon and restricted eye movements occurred. Mean corneal epithelial repair time in stem cell group was （5.7 ± 1.3）d,while （6.1± 1. 1）d in eonjunctival group. Pterygium recurrence rate was 4.9% （2/41）, while 9.8% （4/41） in conjunctival group. The difference between the two groups was not significant （ P 〉 0.05 ）. Conclusions Retrograde pterygium excision eombined with autologous conjunc- tival or corneal limbal stem cell transplantation can effectively reduce the recurrence rate of pterygium. （ Chin J Ophthaltool and Otorhinolaryngo1,2010,10：303-305 ）
