大鼠大脑中动脉局灶性脑缺血时间与梗死体积的相关性

摘    要:背景缺血性脑卒中能否得到有效的保护性治疗与把握治疗时间窗有密切关系,因此了解脑梗死演变的自然过程具有重要意义.目的探讨大鼠大脑中动脉局灶性脑缺血模型时间与梗死体积的关系.单位解放军济南军区总医院神经内科.设计随机对照实验研究.材料实验在济南军区总医院全军神经内科中心动物实验室完成.选择雄性Wistar大鼠54只,根据缺血时间分为缺血1,1.5,2,3,4,6,9,12,24 h组,每组6只.干预采用鼠须聚乙烯醇栓塞大鼠大脑中动脉,观察缺血1,1.5,2,3,4,6,12及24 h用红四氮唑标记的梗死体积,并与时间进行相关分析.主要观察指标脑缺血不同时间时梗死体积.结果缺血1 h组及1.5 h组TTC染色未发现梗死灶,缺血2 h大鼠基底核区出现梗死灶,边界欠清,4 h缺血动物梗死体积明显增大,和2,3 h组比较有显著差异(P<0.01).各组动物随时间梗死体积渐趋增大,与时间有显著相关性(r=0.86,P<0.01).至12 h梗死体积渐趋稳定,与24 h点相比无显著性差异,但24 h组动物死亡率高.结论大鼠大脑中动脉永久性闭塞性脑缺血模型,梗死体积出现的最小时间点可能为2 h,体积随时间进行性增大,至12 h基本趋于稳定.

Abstract:BACKGROUND: Whether ischemic cerebral apoplexy can get effective treatment is closely related to the time window of treatment. Therefore, it is very important to understand the natural evolutive procedure of cerebral infarction.OBJECTIVE: To explore the relationship between the ischemic time and infraction volume.SETTING: Department of Neurology, General Hospital of Jinan Military Area Command of Chinese PLA.DESIGN: A randomized controlled study.MATERIALS: The experiment was completed in the Central Animal Laboratory of Neurological Department of General Hospital of Jinan Military Area Command of Chinese PLA. A total of 54 Wistar male mice were chosen and divided into 9 groups: 1 hour, 1.5 hours, 2 hours, 3 hours, 4 hours, 6 hours, 9hours, 12 hours and 24 hours according to ischemic time, each with 6 mice.INTERVENTIONS: Mouse whisker with polyvinyl alcohol was used to embolized the middle cerebral artery to observe the infracted volume within 1,1.5, 2, 3, 4, 6, 12, 24 hours after ischemia markered by triphenyltetrazolium chloride(TTC), and conduct correlative analysis with time.MAIN OUTCOME MEASURES: The infracted volume in different time of ischemia.RESULTS: There was no infracted area in lhour group and 1.5 hours group after ischemia while there appeared infracted areas with unclear boundary in the basal ganglia in mice of 2 hours group. The infracted volumes greatly increased in the 4 hours group mice with significant difference between it and2 hours and 3 hours groups( P < 0. 01 ) . The infrscted volumes gradually increased of each group with the advance of time and showed significant correlation with time ( r = 0. 86, P < 0.01 ). The infracted volume turned to stable till the 12th hour and there was no difference on it between 12 hours and24 hours, however, the death rate of mice was highest in 24 hours group.CONCLUSION: In the permanently occlusive cerebral ischemia model of middie cerebral artery of mice, the possibly smallest time point when infracted area appears is 2 hours. The infracted volume increases with the advance of time and tend to be stale in the 12th hour.
