自体肋骨移植在下颌骨缺损修复中的应用

目的：下颌骨缺损的修复旨在重建下颌骨的完整性，恢复下颌骨功能和口腔功能，矫正颌面畸形。方法：按常规切除病变的下颌骨节段；取适宜自体肋骨、肋软骨游离移植颌骨缺损，骨接端用不锈钢丝结扎固定，并采用颅颌绷带辅助固位。结果：８０例治疗患者中，植骨成骨、修复效果满意者７８例，占９７．５％（７８／８０）；植骨块最终取出，植骨失败２例，占２．５％（２／８０）；植骨床感染１１例，占１３．７５％（１１／８０）；结论：自体肋骨软骨游离移植仍不失为修复下颌骨缺损较理想的方法。

RECONSTRUCTION OF THE MANDIBULAR DEFECTS USING AUTOGENOUS RIB GRAFT

Objective:The purpose for the repair of mandibular defect is to reconstruct the mandibular intergration,resume functions of mandible and oral cavity,and correct deformities of maxilloface. Method:The diseased mandible was regular excised,then free autgenous rib or rib-cartilage graft was abtained and transplanted in the defect at the same operation.The graft was fixed with stainless wire and added with craniomaxillary bandage. Result:The success rate of transplantations in 81 cases was 97.5%(78/80);while the failure rate was 2.5%(2/80);the normal healing rate and the infected rate of implanted bony beds were 86.25%(69/80)and 13.75%(11/80). Concolusion:Free autogenous rib or rib-cartilage graft applied in the construction of the mandibular defects is still an alternative approach.
