摘    要:目的探讨CT导引下突出物内及盘内联合注射胶原酶治疗腰椎间盘突出症的临床疗效。方法对143例腰椎间盘突出症的患者采取CT导引下注射胶原酶于突出物内及盘内,术后随访3个月~1a,观察其疗效。结果疗效优者49例。良者83例,可者7例,差者4例,优良率92.3%。随访期间32例进行了CT复查,其中15例原椎间盘突出物明显缩小或消失,14例有不同程度的缩小,无明显改变者3例,1例并发椎间盘炎。结论CT导引下突出物内及盘内联合注射胶原酶治疗腰椎间盘突出症具有定位准确,优良率高等优点,是一种安全。有效的方法。

Abstract:Objective] To investigate the clinical effects of injecting eollagenase into the prominence and intradiscal collagenase under CT-guided to treat lumbar disc herniation. Methods] One hundred and forty-three patients with lumbar disc herniation were treated with injecting collagenase into the prominence and intradiscal colla- genase under CT-guided, following up three to twelve months after operation to observe the effects. Results] 49 patients completely recovered, 83 patients significantly improved, 7 patients partially responsed to the treatment while 4 patients did not have any responsed to the treatment, The effective rate to the treatment is 92.3%. The herniation disappeared or significantly reduced as demonstrated by CT scan in 15 of 32 patients, partially reduced in 14 patients, no changes existed in 3 cases. There was 1 case of disc inflammation in all the patients during follow up period. Conclusion] Inject collagenase into the prominence and intradiscal collagenase under CT-guided is a safe and effective method in the treatment of herniated lumbar disc. It has the advantage of accurate localization and high rate of respones.
