MR增强FLAIR序列对脑转移瘤的诊断价值

目的探讨MRI增强液体衰减反转恢复(FLAIR)序列对脑转移瘤的诊断价值。方法 45例患有原发性恶性肿瘤且临床怀疑有脑转移的患者。行常规MR检查平扫和增强,在增强后加扫FLAIR序列,图像由2名有经验的放射科医师评价。结果 45例患者中30例发现脑转移,共计191个病灶,增强常规T1WI显示179个;增强FLAIR显示185个,两种序列上转移瘤的病灶/正常组织信号强度比差异有统计学意义(p0.01)。结论增强FLAIR序列是增强常规T1WI的有效补充,它可以有效的显示脑转移瘤,尤其对颅底及浅表皮质转移瘤的检出和诊断更具优势。增强FLAIR与常规T1WI结合可以提高脑转移瘤的检出率和诊断准确性。

Value of MRI contrast-enhanced FLAIR sequences in the diagnosis of metastatic encephaloma

Objective To evaluate the diagnostic value of contrast-enhanced fluid attenuated inversion recovery（FLAIR） sequences in detection of metastatic encephaloma.Methods A total of 45 patients with primary malignancy and suspected cranial metastases underwent MR examinations.In addition to routine plain scans and contrast-enhanced scans,FLAIR was performed after contrast administration.The postcontrast images were reviewed by two experienced radiologists.Results There were 35 patients found magnetic emcephaloma among the 45 patients,191 lesions were displayed,179 lesions were displayed on postcontrast T1WI,and 185 lesions on postcontrast FLAIR.The signal ratios of lesion had significant difference in two sequences.Conclusion Postcontrast FLAIR is an effective complement to postcontrast T1WI.It can effectually detect magnetic emcephaloma,and is especially useful for detecting superficial lesions.The detection rate and diagnosis accuracy of intracranial metastases could be improved by using T1WI combined with FLAIR sequences after contrast enhancement.
