中医清补法对创伤后全身炎症反应综合征的疗效研究

目的:观察中医清补法对创伤后全身炎症反应综合征(SIRS)的临床疗效。方法:将48例SIRS患者随机分为治疗组和对照组,对照组采用处理原发病灶、抗休克、氧疗、预防感染、止血及其他必要的支持治疗措施,治疗组在对照组基础上给予清补中药复方水煎溶液经口服或胃管注入,于治疗1 d,3 d,5 d,7 d后,记录两组患者生命体征变化及动态检测血液PCT,IL-6,IL-8含量变化,并进行比较。结果:治疗组总有效率(66.67%)显著高于对照组(45.83%),P0.05;与对照组比较,治疗1 d和3 d后,血液IL-6,IL-8和PCT均无显著性下降,但是,随用药时间延长,治疗5 d和7 d后IL-6,IL-8和PCT均显著低于对照组,P0.05。结论:清补法治疗后,有助于降低血液细胞炎症因子水平,以实现减轻创伤后全身炎症反应,从而对创伤后SIRS有一定的疗效。

Study on the Clinical Effect of Qingbu Therapy of Traditional Chinese Medicine in the Treatment of Post-Traumatic Systemic Inflammatory Response Syndrome

Objective:To study the clinical effect of Qingbu therapy of traditional Chinese medicine in the treatment of post- traumatic systemic inflammatory response syndrome. Methods:Forty- eight systemic inflamma-tory response syndrome cases were divided randomly into control group(n=24) and treatment group(n=24).Treatments including treating primary lesion,anti- shock,oxygen therapy,infection prevention,hemostasis,and other necessary supportive measures were administrated in the control group,while,Qingbu formulas were administrated through gavage in treatment group besides the treatment in the control group. On the 1st day,3rd day,5th day and 7th day,vital signs were recorded,and blood PCT,IL-6 and IL-8 level were detected.Results:Compared with the control group(45.83%),the effectiveness increased significantly in the treatment group(66.67%)(P<0.05). On the 1st day and 3rd day treatment,plasma PCT,IL-6 and IL-8 level were of no significant differences,but on the 5th day and 7th day,plasma PCT,IL-6 and IL-8 level decreased significantly(P<0.05). Conclusions:Qingbu therapy of traditional Chinese medicine could contribute to reducing blood cell inflammatory factors level,and thus it relieve the post- traumatic systemic inflammatory response and improve clinical efficacy.
