冬令膏方对慢性阻塞性肺疾病患者炎症标志物的影响

摘    要:目的:观察冬令膏方对慢性阻塞性肺疾病(COPD)患者血清凝血因子Ⅰ(FIB)、超敏C-反应蛋白(CRP)、趋化细胞因子-18(CCL-18)、肺泡表面活性物质相关蛋白-D(SP-D)的影响,探讨冬令膏方治疗该病的可能有效机制。方法:76例COPD患者纳入研究,采用简单随机化方法分为治疗组40例、对照组36例,对照组予舒利迭50μg/500μg 1吸,日2次,联合噻托溴铵粉吸入剂18μg 1吸,日1次,常规治疗。治疗组在对照组治疗的基础上加用冬令膏方,比较两组急性加重风险炎症指标FIB、CRP、CCL-18、SP-D的变化情况。结果:两组患者CRP、FIB变化差异无统计学意义(P0.05),治疗12个月后治疗组CCL-18的水平低于对照组(P0.05),两组SP-D的变化情况比较,治疗组改善优于对照组(P0.05)。结论:冬令膏方治疗可能从改善SP-D的机制上减少其急性加重风险。

Effects of Winter Soft Extract on the Inflammatory Markers of Patients with Chronic Obstructive Pulmonary Disease

Abstract:Objective:To observe the effects of winter soft extract on Chemokine(C-C motif) ligand 18(CCL-18),C-reactive protein(CRP),pulmonary surfactant-associated protein D(SP-D) and fibrinogen(FIB) in the serum of the patients with chronic obstructive pulmonary disease(COPD) in stable phase. Methods:Total76 patients were randomly divided into the control group and treatment group(n=40 in treatment group,n=36 in control group),conventional inhalation therapy(Seretide 50 μg/500 μg 1 inhalation twice a day and Thiamethoxam bromide powder inhaler 18 μg 1 inhalation once a day) was provided for the two groups,while oral winter soft extract was supplied only to the treatment group. The changes of inflammatory markers including CCL-18,SP-D,CRP,FIB of the two groups were compared. Results:The changes of CRP,FIB of the two groups had no statistical significance in the two groups(P>0.05) while the levels of CCL-18 and the change of SP-D in the treatment group were lower than those in the control group(P<0.05). Conclusion:Treatment with winter soft extract lowered the serum concentration of SP-D to reduce the acute aggravation risk of COPD.
