脑动脉狭窄闭塞患者侧支循环建立的相关影响因素

目的探讨脑动脉狭窄或闭塞患者侧支循环建立相关危险因素。方法脑动脉狭窄或闭塞患者175例,均行全脑血管造影术,分析脑动脉狭窄程度与侧支循环建立的关系及其影响因素。结果侧支循环代偿良好率随脑动脉狭窄程度的增加而升高。单因素分析结果显示代偿良好组吸烟、糖尿病、高脂血症、高血压比例和纤维蛋白原水平明显高于代偿不良组（P〈0.05）;多因素logistic回归分析结果显示,吸烟,合并糖尿病、高脂血症、高血压,纤维蛋白原是脑动脉狭窄侧支循环的危险因素。结论侧支循环代偿良好率随脑动脉狭窄程度的增加而升高;吸烟,合并糖尿病、高脂血症、高血压,纤维蛋白原是脑动脉狭窄侧支循环的危险因素。

Related Influencing Factors for Collateral Circulation Formation in Patients with Cerebral Arterial Stenosis/Occlusion

Objective To investigate the related risk factors for collateral circulation formation in patients with cerebral arterial stenosis / occlusion. Methods One hundred and seventy-five patients with cerebral arterial stenosis / occlusion underwent digital subtraction angiography（ DSA）. The relationship between the degree of cerebral arterial stenosis and collateral circulation formation,and the influencing factors were analyzed. Results The excellent rate of collateral circulation compensation increased with the increasing severity of cerebral arterial stenosis. The results of univariate analysis showed that there were significantly higher ratioes of smoking,diabetes mellitus,hyperlipemia,hypertension and higher level of fibrinogen in the excellent compensation group compared with decompensation group（ P〈0. 05）. The results of multivariate logistic regression analysis showed that smoking,being complicated with diabetes mellitus,hyperlipemia,hypertension and higher level of fibrinogen were the risk factors for collateral circulation formation in patients with cerebral arterial stenosis. Conclusion The collateral circulation compensation is improved more significantly with the increasing severity of cerebral arterial stenosis. Smoking,being complicated with diabetes mellitus,hyperlipemia,hypertension and higher level of fibrinogen are risk factors for collateral circulation in patients with cerebral arterial stenosis.
