小儿轮状病毒肠炎并发心肌损害危险因素分析

摘    要:目的:分析小儿轮状病毒(rotavirus,RV)肠炎并发心肌损害(myocardial damage,MD)的危险因素,以指导临床诊疗。方法收集207例RV肠炎患儿的临床资料,对可能导致MD的变量进行单因素及多因素Logistic回归分析。结果207例RV肠炎患儿中,合并MD 112例(54.1%);单因素分析显示:年龄≤1岁、中度及重度脱水、酸中毒、心电图异常是小儿RV肠炎并发MD的危险因素;多因素Logistic回归分析显示:年龄≤1岁是小儿RV肠炎并发MD的独立危险因素。结论有中度或重度脱水、酸中毒、心电图异常的RV肠炎患儿,尤其当年龄≤1岁时,需重点监测心肌损害情况,并及时给予保心等干预治疗,避免发生严重后果。

Abstract:Objective To analyze the risk factors in children with rotavirus ( RV) enteritis complicated with myocardial dam-age ( MD) in order to guide the clinical diagnosis and treatment .Methods The clinical data from 207 cases of RV enteritis were col-lected.Variables that may lead to MD were analyzed by using univariate and multivariate logistic regression analysis .Results Of 207 children with RV enteritis ,112 cases were complicated with MD ( 54.1%) .Single factor analysis showed that age (≤1 years old ) , moderate and severe dehydration ,acidosis and ECG abnormalities were risk factors for RV enteritis complicated with MD .Multivariate logistic regression analysis showed that age (≤1 years old) was an independent risk factor for RV enteritis complicated with MD .Con-clusion RV enteritis children who have moderate or severe dehydration ,acidosis and abnormal ECG ,especially ≤1 years old ,need to be monitored for myocardial injury .At the same time,a heart intervention therapy should be given to avoid the occurrence of serious consequences .
