双源 CT 血管成像技术与彩色多普勒超声诊断颈动脉粥样硬化性疾病的对照研究

摘    要:目的:探讨并比较双源CT血管成像技术( dual-Source CT angiography ,DSCTA)与彩色多普勒超声( color doppler flow imaging,CDFI)在颈部动脉粥样硬化性疾病诊断中的价值。方法回顾性分析76例临床怀疑颈动脉粥样硬化患者的颈动脉DSCTA及CDFI影像学资料,比较两种影像学方法检出的颈动脉斑块部位、数目、性质及血管狭窄程度有无差异性。结果76例患者中,DSCTA发现40例有颈动脉斑块、CDFI发现60例有颈动脉斑块,CDFI发现斑块数目多于DSCTA,但仅对颈总动脉分叉处斑块的检出率差异有统计学意义(P<0.05)。 DSCTA检出钙化斑块数目多于CDFI,差异有统计学意义(P<0.05), CDFI检出混合斑块及软斑块数目多于DSCTA,差异有统计学意义(P<0.05)。 DSCTA检出颈动脉轻度狭窄例数多于CDFI,差异有统计学意义( P<0.05)。结论 DSCTA与CDFI在颈动脉粥样硬化性病变的诊断中各有优势及局限性,二者互相补充可以提高诊断的准确性。

A comparative study on diagnosis of carotid artery atherosclerosis disease by using dual-source CT angiography or color Doppler ultrasound

Abstract:Objective To compare the diagnostic value of dual-source CT angiography ( DSCTA) with color Doppler flow ima-ging ultrasound ( CDFI) for carotid artery atherosclerosis disease .Methods DSCTA and CDFI imaging data of 76 cases suspected with carotid artery atherosclerosis disease were analyzed retrospectively .Location,amount and characteristics of atherosclerosis plaque and degree of vascular straitness of carotid artery that were detected by DSCTA or CDFI examination were compared .Results In all 76 ca-ses,atherosclerosis plaques were observed in 40 cases by DSCTA and in 60 cases by CDFI.Although amount of atherosclerosis plaques found by CDFI was more than that by DSCTA ,the difference was statistically significant only at bifurcation section of carotid common artery(P 〈0.05).Amount of calcific plaques discovered by DSCTA was more than that by CDFI (P〈0.05).Amount of mixed plaques and soft plaques found by CDFI was more than that by DSCTA ( P〈0.05 ) .Cases with slight straitness of carotid artery discovered by DSCTA were more than that by CDFI ( P〈0.05 ) .Conclusion DSCTA and CDFI have respective advantages and shortcomings in the diagnosis of in carotid artery atherosclerosis disease .The two complement each other can improve the accuracy of diagnosis .
