老年骨质疏松症并发股骨颈骨折的术后护理及患者自我感受负担程度的调查研究

目的：探讨老年骨质疏松症并发股骨颈骨折的术后护理及患者自我感受负担程度。方法选取92例老年骨质疏松症并发股骨颈骨折术后患者，按照随机数字表法分为两组，对照组（45例）采用常规护理，观察组（47例）在常规护理基础上对术后心理、饮食、康复和并发症进行护理干预，比较两组患者护理效果；采用自我感受负担量表（ SPBS）和生活质量评分量表（ QOLS）对患者护理前后自我感受负担程度和生活质量水平进行评估。结果观察组护理干预后SPBS评分较护理干预前明显降低，QOLS评分明显增高，骨折总愈合率、护理满意率，以及术后1、3个月髋关节功能评分明显高于对照组，差异有统计学意义（ P＜0.05）；Pearson相关分析发现：SPBS评分与QOLS评分呈明显的负相关（ r＝-4.57，P＜0.05）。结论术后护理干预有助于促进老年骨质疏松症并发股骨颈骨折术后骨折愈合和髋关节功能康复，降低自我感受负担程度，提高护理质量和患者的生活质量。

Investigation of postoperative nursing and self perceived burden degree of senile osteoporosis complicated with transcervical fracture

Objective To investigate the postoperative nursing and self perceived burden condition of senile osteoporosis com -plicated with transcervical fracture .Methods Ninety-two senile postoperative patients with osteoporosis complicated with transcervical fracture were randomly divided into control （n=45） and observation （n =47） groups.The control group was given routine nursing while the observation group was given intervention measures such as postoperative psychological , diet, rehabilitation and complications nursing on the base of control group .The nursing effect was compared between the two groups .Meanwhile , self perceived burden （SPB） condition and quality of life before and after care were assessed by using SPB scale （SPBS） and quality of life scale （QOLS）. Results The total fracture healing rate and nursing satisfaction rate in the observation group were significantly higher than that in the control group （ P〈0.05 ） .The hip joint function score in 1 and 3 months after operation in the observation group were significantly higher than that in the control group （ P〈0.05 ） .The SPBS score was significantly decreased while QOLS score was obviously increased after nursing intervention when compared to before treatment （ P〈0.05 ） .The Pearson correlation analysis showed that SPBS score and QOLS score were negatively correlated （ r=-4.57 , P〈0.05 ） .Conclusion Postoperative nursing intervention is helpful to senile os-teoporosis complicated with transcervical fracture .It can promote the rehabilitation of hip function , reduce self perceived burden , and improve the nursing quality and life quality .
