E-钙粘蛋白表达与胃癌临床病理学关系的研究

目的：探讨E-钙粘蛋白（E—cadherin，E—cad）的表达与胃癌的浸润、转移及预后的关系。方法：应用SP免疫组化方法检测76例胃癌E—cad的表达。结果：正常胃粘膜上皮中E—cad全部表达。Ⅰ期E—cad的异常表达率为19．6％，而Ⅱ、Ⅲ、Ⅳ期的异常表达率分别是57．2％、62．1％和69．3％，Ⅰ期和Ⅱ、Ⅲ、Ⅳ期间有非常显著性差异（P〈0．01）。伴有淋巴结转移者51例中异常表达为74．3％，而无淋巴结转移者40．2％，两者间有非常显著性差异（P〈0．05）。在进展期胃癌中按肿瘤生长方式分组，结节型E—cad异常表达率41．2％，而溃疡浸润型异常表达率67．3％，两者间有显著性差异（P〈0．05）；高中分化胃癌E—cad异常表达率32．0％，而低分化胃癌E—cad异常表达率69．6％，两者间有非常显著性差异（P〈0．01）。癌细胞浸润深度达粘膜、粘膜下层和肌层者E—cad异常表达率为30．1％、38．2％，而浸润至浆膜及浆膜外者异常率为67．3％，两者间有非常显著性差异（P〈0．01）。胃癌组织中E—cad的表达与癌组织分化、浸润和转移密切相关。结论：E—cad与肿瘤的发生、浸润、转移有关，可作为一种新的肿瘤标记物，对判断预后有一定价值.

Relationship Between Expression of E-cadherin in Gastric Carcinoma and its Clinicopathological Features

Objective:To study the relationship between the expression of E-cadherin and invasion,metastasis and progression of human gastric cancer.Methods:76 cases of human gastric cancer were examined for E-cadherin by SP immunohistochemistry.Results:E-cadherin was present in all normal gastric mucomembranous epithelium.E-cadherin was associated with gastric cancer differentiation,invasion and metastasis (P<0.01).Conclusion:E-cadherin is associated with gastric cancer differentiation,invasion and metastasis,which suggests E-cadherin as a new tumor marker.
