摘    要:目的探讨Z形皮瓣联合纤维蛋白胶对乳腺癌术后皮下积液的预防作用。方法观察组74例,采用Z形皮瓣,术中创面喷洒纤维蛋白胶;对照组36例,采用传统纵形切口,创面不喷洒纤维蛋白胶,腋窝常规放置引流管3~4 d。结果观察组术后皮下积液、患侧上肢水肿发生率为1.4%,明显低于对照组的11.1%(P<0.05)。观察组术后患侧上肢功能受限发生率为0%,明显低于对照组的22.2%(P<0.05)。结论Z形皮瓣联合纤维蛋白胶创面喷洒有利于乳腺癌术后皮下积液的预防。

Abstract:Objective To explore the application effects of Z-shape flap combined with fibrin glue in prevention of subcutaneous hydrops in breast radical mastectomy.Methods 74 breast cancer cases were arranged as observation group who were underwent transferring suture of Z-shape flap and spraying fibrin glue on the wound surface.36 cases as control group who were performed vertical incision and placed a drainage tube in axilla.Results The incidence of post-operative subcutaneous hydrops and operation-side upper extremity edema was 1.4% and the incidence of functional limitation in illness-side upper limb was 0% in observation group,both were lower than that 11.1% and 22.2% of control group respectively(P<0.05).ConclusionsZ-shape flap transferring suture combined with fibrin glue spraying on wound is effective in prevention of post-operative subcutaneous hydrops.
