出血性脑梗死及其相关危险因素的临床分析

目的探讨出血性脑梗死的临床特点和危险因素。方法采用回顾性分析方法,将2003年1月~2006年1月收治的1 548例脑梗死患者分为出血性脑梗死组(52例)及对照组(1 496例),采用logistic回归模型分析出血性脑梗死的危险因素。结果出血性脑梗死占同期脑梗死患者的3.36%,脑栓塞、皮质梗死、大面积脑梗死、高血糖为出血性脑梗死的独立危险因素;抗凝、溶栓治疗不增加出血的危险性,但容易形成局部血肿,出血性脑梗死患者的预后优于对照组;结论脑栓塞、皮质梗死、大面积脑梗死、高血糖容易发生出血性脑梗死,出血性脑梗死患者的短期预后与原有梗死灶大小及部位有关,优于对照组,大量出血形成血肿预后不良。

Study on hemorrhagic cerebral infarction and the risk factors

Objective To study the clinical feature and the risk factors of hemorrhagic cerebral infarction.Methods The group of cases with hemorrhagic cerebral infarction(52cases) and the control group(1496 cases) were retrospectively analysed.Logistic regression analysis was employed to find out the risk factors of hemorrhagic cerebral infarction and the clinical features.Results The percentage of hemorrhagic cerebral infarction was 3.36% of cerebral infarction cases.Cerebral embolism,involvement of cerebral cortex,big area of cerebral infarction,hyperglycemia were the independent risk factors for the hemorrhagic cerebral infarction.The antithrombotic and anticoagulation therapy did not increase the risk of hemorrhagic cerebral infarction,but they were apt to cause hematoma formation.The prognosis of hemorrhagic cerebral infarction was better than that of the control group.Conclusion The hemorrhagic cerebral infarction does not influence the prognosis,which is related to the location and volume of the cerebral infarction,but once the hematoma is formed,the prognosis is poor.
