冠状动脉搭桥术前吸气肌锻炼的临床研究

摘    要:目的评价冠状动脉搭桥手术(CABG)术前吸气肌锻炼对改善患者肺功能、预防术后肺部并发症的作用.方法回顾性分析238例CABG病人,186例进行术前吸气肌锻炼的病人作为实验组,52例未进行吸气肌锻炼的病人作为对照组.比较分析两组病人在术前、术后的血气变化,术后机械呼吸支持时间,肺部感染发生率.结果实验组术后血气指标的下降明显低于对照组,术后机械呼吸支持时间和肺部感染发生率均低于对照组.结论 CABG术前吸气肌锻炼能明显改善患者术前肺功能,缩短术后机械呼吸支持时间,减少术后肺部并发症的发生.

Abstract:Objective To evaluate the efficiency of inspiratory muscle training to improve respiratory function and prevent pulmonary complications after coronary artery bypass graft(CABG).Methods 238 patients who underwent CABG between May 1997 and December 2000 were assessed retrospectively.The patients were divided into two groups:186 patients underwent inspiratory muscle training before the CABG assigned to the training group,52 patients without muscle training were assigned to the control group.The parameters were measured and analyzed.Results In the training group,there were shorter time of mechanical ventilation and lower rate of postsurgical pulmonary complications than the control group(P<0.05).There was a significant decrease in the parameters of arterial blood gases in the control group following CABG.Conclusion Inspiratory muscle training before CABG can improve respiratory function,decrease postsurgical pulmonary complications and shorten the time of mechanical ventilation.
