微创伤显微手术治疗高血压脑出血

摘    要:目的探讨高血压脑出血外科治疗方法和技巧。方法对我院 1997年 1月至 2 0 0 2年 4月行小骨窗微创伤显微神经外科手术治疗的 5 6例高血压脑出血病例作一回顾性分析。结果术后 8月行格拉斯哥结果评分 (GOS) ,痊愈 8例 (14 .3% )中残 32例 (5 7.4 % ) ,重残 9例 (16 .1% ) ,植物状态 0例 ,死亡 7例 (12 .5 % )。死亡病例中 ,3例晚期脑疝患者死于术后并发症 ,4例死于术后几天的再出血。结论小骨窗显微神经外科手术治疗高血压脑出血损伤小、血肿清除彻底、止血可靠、疗效较好。

Abstract:objective To explore the approach and skill of surgery for hypertensive intracerebral hemorrhage. Methods 56 cases of hypertensive intracerebral hemorrhage operated on with small craniectomy and microinjurous surgery in our hospital during a period from January 1997 to April 2002 were analyzed retrospectively. Results With 8 months follow-up study after surgery and according to Glasgow Outcome Score(GOS), the cases of good recovery(GR), moderate disability(MD), severe disability(SD), persistent vegetative state(PVS) and dead (D) were 8 cases (14.3%), 32 cases (57.4%), 9 cases (16.1%), zero and 7 cases (12.5%) respectively. Among the dead cases ,3 cases with late cerebral herniation died of complications, while the rest died of intracerebral re-bleeding in several days after surgery. Conclusion Small craniectomy and microinjurous surgery for hypertensive intracerebral hemorrhage is a surgical method with advantages of slight trauma to brain tissue, complete evacuation of clot, stoping bleeding properly ,and improved effect.
