骨髓增生异常综合征与再生障碍性贫血患者骨髓细胞免疫表型比较分析

摘    要:目的探讨骨髓细胞免疫表型对骨髓增生异常综合征(MDS)与再生障碍性贫血(AA)的临床鉴别诊断价值。方法采用单克隆抗体-生物素-亲和素酶标法对本院4年内初诊未治疗的23例AA患者、19例MDS患者及50例缺铁性贫血患者(对照组)骨髓细胞免疫表型进行检测,统计分析CD3、CD5、CD10、CD13、CD19和CD33抗原在三种疾病中表达差异。结果 AA患者骨髓细胞CD3和CD5抗原表达率明显高于对照组和MDS组(P<0.05),CD10、CD13和CD33抗原表达在AA组与对照组间无差异(P>0.05)。MDS患者骨髓细胞CD13和CD33抗原表达率均明显比对照组和AA组高(P<0.05),CD19表达低于AA组和对照组,CD3抗原表达在MDS组与对照组间差异无统计学意义(P>0.05)。结论骨髓细胞免疫表型对于AA与MDS的鉴别有一定参考价值。

Abstract:Objective To investigate the differential diagnosis of immunophenotype of bone marrow cells from patients with myelodysplastic syndromes and aplastic anemia.Methods Selection of CD3,CD5,CD10,CD13,CD19,CD33 monoclonal antibody,23 patients with newly diagnosed untreated AA,19cases with MDS,and 50 cases with iron deficiency anemia(the control group),the bone marrow cell immunophenotypes were analyzed to find the antigen differentially expressed in different diseases with biotin-avidin enzyme linked immunosorbent assay,Results CD3 and CD5 antigen expression in the patients with AA was higher than the control group and patients with MDS(P<0.05).CD10,CD13 and CD33 antigen expression in the patients with AA and control groups showed no differences.in the patients with MDS,CD13 and CD33 antigen expression were significantly higher,CD19 expression was significantly lower than the control group and the patients with AA(P<0.05).CD3 expression in the patients with MDS and the control group showed no difference.Conclusion Analysis of bone marrow cell immunophenotype in differentiating MDS and AA is valuable.
