舒芬太尼联合丙泊酚对妇科手术镇痛效果的研究

目的：观察分析舒芬太尼联合丙泊酚在妇科手术术中和术后镇痛中的效果，为临床提供参考。方法：选取288例妇科手术患者作为研究对象，按照随机数字表法分为观察组和对照组各144例，分别给予舒芬太尼联合丙泊酚给药及丙泊酚单独给药，观察两组患者术中和术后镇痛效果。结果：给药前、睫毛反射消失时、术中及术毕时，两组的收缩压（SBP）、舒张压（DBP）、心率（HR）和脉搏血氧饱和度（SpO2）比较差异均无统计学意义（P〉0.05）；观察组麻醉效果明显优于对照组，其中观察组麻醉效果为优所占比例明显高于对照组，差异有统计学意义（χ2=4.72，P=0.03）；观察组术后12、24、36、48 h的VAS评分均明显低于对照组，差异均有统计学意义（P〈0.05）；观察组术后不良反应6例，其中腹痛1例，头晕乏力2例，嗜睡2例，恶心呕吐1例；对照组术后不良反应11例，腹痛2例，头晕乏力2例，嗜睡3例，恶心呕吐4例，经对症处理后均在12 h内好转；观察组满意率及满意度评分均明显高于对照组，差异均有统计学意义（P〈0.05）。结论：丙泊酚与舒芬太尼联合应用能够有效提高术中麻醉效果及术后镇痛效果，是一种安全有效的麻醉方法。

Study on the Analgesic Effect of Sufentanil Combined with Propofol in Gynecologic Operation

Objective：To observe the effect of sufentanil combined with propofol for analgesia in gynecologic operation during and after operation,and to provide reference for clinical. Method：288 cases of gynecologic operation were selected and divided into the control group（n=144）and the observation group（n=144）,according to the random number table method,and were accepted sufentanil combined with propofol administered and propofol administered alone,the analgesic effect of two groups with postoperative were compared.Result：Before drug,loss of eyelash reflex, intraoperative and postoperative time,two groups with systolic pressure（SBP）,diastolic blood pressure（DBP）,heart rate（HR）and pulse oxygen saturation（SpO2）were not significant difference（P〉0.05）;the anesthesia effect of the observation group was better than the control group,anesthetic optimal effect accounts for the proportion the observation group was higher than that in the control group,the difference was statistically significant（ χ2=4.72,P=0.03）;the VAS score in 12,24,36,48 h of the observation group were significantly lower than those in the control group,with a statistically significant differences（P〈0.05）;the adverse reactions of the observation group after operation were 6 cases, included 1 cases of abdominal pain,2 cases of dizziness and fatigue,somnolence in 2 cases,1 cases of nausea and vomiting;the adverse reactions of the control group had 11 cases,2 cases of abdominal pain,dizziness and weakness in 2 cases,3 cases of somnolence,nausea and vomiting in 4 cases,the patients were better by symptomatic treatment in 12 h;the satisfaction rate and satisfaction scores of the observation group were significantly higher than those in the control group,the difference was statistically significant（P〈0.05）.Conclusion：Application of propofol combined with sufentanil can effectively enhance the analgesic effect of anesthesia during operation and postoperative,is a safe and effective anesthetic method.
