曼月乐在子宫瘢痕愈合不良所致月经异常中的个性化护理观察

目的：观察探讨曼月乐在治疗子宫瘢痕愈合不良所致月经异常中的作用。方法：选取2010年1月-2013年12月期间本院收治的子宫瘢痕愈合不良合并月经过多的患者80例作为研究对象，将其按随机数字表法分为观察组和对照组各40例，两组均给予放置曼月乐治疗。对照组采用常规护理，观察组在常规护理的基础上采用个性化护理。并对放置曼月乐后对患者随访1年，并观察记录3、6、12个月时放置曼月乐前后的月经量及血红蛋白含量的变化，放置曼月乐前后的血清性激素水平，并比较两组经期延长的治疗疗效，以及对两组患者的焦虑程度和生活质量进行观察。结果：观察组放置后3、6、12个月时的月经量均较放置前明显减少，血红蛋白则较放置前均明显升高，差异均有统计学意义（P〈0.05或P〈0.001）。观察组放置曼月乐后3、6、12个月时的血清FSH、LH、E2水平较放置前比较差异均无统计学意义（P〉0.05）。观察组的总有效率72.5%（29/40）明显高于对照组的42.5%（17/40），差异有统计学意义（χ2=7.366，P=0.007）。观察组生活质量明显高于对照组，焦虑情况低于对照组，差异均有统计学意义（P〈0.05）。结论：个性化护理措施在曼月乐治疗子宫瘢痕愈合不良所致月经异常中具有良好作用，值得在临床进行推广。

Nursing Observation of Mirena in Uterine Scar Poor Healing Caused by Abnormal Menstruation

Objective：To observe and investigate the effect of Mirena in uterine scar poor healing caused by abnormal menstruation.Method：80 patients with uterine scar poor healing and menorrhagia in our hospital from January 2010 to December 2013 were selected as research objects,they were randomly divided into the observation group and the control group,40 cases in each group.The two groups were given the Mirena treatment.The control group received routine nursing care,the observation group was given personalized nursing on the basis of routine nursing care.The changes of menstrual quantity and the content of hemoglobin and the serum sex hormone levels changes in the observation group after using Mirena 3,6,12 mouths were observed and recorded.The treatment efficacy of menstrual extension between the two groups were compared.And the two groups of patients with anxiety and quality of life were observed.Result：The menstrual quantity of the observation group after using Mirena 3,6,12 months were significantly less than before using Mirena, and hemoglobins were significantly higher than before using Mirena,the differences were statistically significant（P〈0.05 or P〈0.001）.There were no significant differences in Serum FSH,LH,E2 levels of observation group（P〉0.05）. The total effective rate of observation group was 72.5%（29/40）,it was significantly higher than 42.5%（17/40）of the control group（χ2=7.366,P=0.007）.The scores of quality of life and anxiety in the observation group were significantly better than those in the control group,the differences were statistically significant（P〈0.05）.Conclusion：Personalized nursing measures in the treatment of uterine scar healing Mirena has good effect caused by abnormal menstruation,worthy of promotion in clinical.
