阻塞性睡眠呼吸暂停低通气综合征患者心律失常和ST段改变的研究

目的 探讨阻塞性睡眠呼吸暂停低通气综合征（obstrutive sleep apnea—hypopnea syndrome，OSAHS）与心律失常和ST段压低的关系。方法 对经多导睡眠仪监测（polysomnography，PSG）确诊的OSAHS 38例行24h动态心电图（holter）观察及夜间尿儿茶酚胺测定，同时选择20例正常人作为对照组。结果 OSAHS组患者体重、呼吸暂停低通气指数（apnea hypopne index，AHI），最低血氧饱和度（lowest saturation，LsaO2）和夜间尿儿茶酚胺与对照组差异有显著性（P〈0．05，P〈0．01）；OSAHS组心律失常发生率为52．63％。ST段压低发生率63．15％，并且随OSAHS病情加重，心律失常，ST段压低发生率升高。结论 OSAHS患者心律失常的发生与呼吸暂停低通气指数。低氧血症及交感紧张密切相关。

Clinical analysis of cardiac arrhythmia and myocardial ischemia in patients with obstructive sleep syndrome

[Objective] To explore the relationship between obstructive sleep apnea-hypopnea syndrome (OSAHS) and arrhythmia and myocardial ischemia. [Methods] 38 patients with OSAHS and 20 of the normol control group were analyed by 24 hours holter. [Results] The weight, apnea hypopnea index (AHI), lowest saturation (Lsao2) and catecholamine in OSAHS group were significantly higher than those of the control group. The incidence of cardiac arrhythmia in OSAHS group were 52.63%, depress of ST in OSAHS group were 63.15% and were increased with the severity of OSAHS. [Conclusion] The occurrence of cardiac arrhythmia in patients with OSAHS is closely related with AHI and nocturnal hypoxemia and sympathetic tone.
