超声彩色多普勒对糖尿病眼部血管血流动力学的研究

摘    要:目的:探讨2型糖尿病患者眼底动脉血流动力学变化与糖尿病视网膜病变发生发展的关系。方法:应用彩色多普勒超声检测75例(150只眼)2型糖尿病患者视网膜中央动脉(CRA)、睫状后动脉(PCA)、眼动脉(OA)的最大收缩期血流速度(PSV)、舒张末期血流速度(EDV)、阻力指数(RI),按眼底病变分为无视网膜病变(NDR)、视网膜病变非增殖期(NPDR)、视网膜病变增殖期(PDR)3组,并与正常对照组53例(106只眼)进行对照。结果:病变组眼部血管CRA、PCA、OA的PSV、EDV呈进行性下降趋势,RI呈进行性升高趋势。NDR组CRA、PCA、OA的EDV及RI与对照组相比差别有统计学意SL(P〈0.05)。结论:彩色多普勒超声能较准确反映糖尿病眼部血管改变,对早期发现视网膜血流动力学改变有重要价值。

Abstract:Objective: To investigate the correlation between the ocular hemodynamic changes and the development of diabetic retinopathy in .type 2 diabetic patients. Methods: We measured the peak systolic velocity (PSV),the end diastolic velocity (EDV) and the resistance index (RI) of central retinal artery (CRA), posterior ciliary artery (PCA) and ocular artery (OA) with color Doppler uhrasonography in 75 type 2 diabetic patients (150 eyes). All of 150 eyes was divided into NDR group, NPDR group and PDR group, and compared with the normal control group. Results: With the progression of retinopathy, PSV and EDV of CRA,PCA and OA decreased and RI increased significantly. The hemodynamic abnormality appeared before retinopathy. Between NDR and normal control group, there was statistical deference in EDV and RI of CRA, PCA and OA (P 〈0.05). Conclusion: Color Doppler uhrasonography can reflect accurately the change of diabetic retinopathy .It also has important value for detecting early retinal hemodynamic changes.
