电视胸腔镜与腹腔镜食管癌根治术的临床应用

目的:评价电视胸、腹腔镜食管癌根治术的临床应用价值。方法:对27例患者行电视胸、腹腔镜食管癌根治术,游离食道应用经右侧胸腔电视胸腔镜手术,游离胃应用经腹腹腔镜手术,食管胃左颈部吻合器吻合。结果:27例患者无围手术期死亡,无术后严重并发症发生。手术共清扫淋巴结225枚,随访至2011年10月全部生存。结论:电视胸、腹腔镜食管癌根治术安全、可行,具有创伤小、出血较少、术后疼痛轻、并发症少、恢复快等优点,有推广价值。

Application of video assisted thoracic surgery and laproscope for radical operation of esophageal cancer

Objective:To summarize the clinical experience of video-assisted thoracoscopic and laproscopic esophagectomy for radical operation of esophageal cancer.Methods:Video-assisted thoracoscopic and laproscopic esophagectomy was performed in 27 patients.Video-assisted thoracoscopic approach was used to mobilize the intrathoracic esophageal and stomach was mob ilized by laparoscope approach.Esophago-gastric anastomosis was performed in the left side of the neck.Results:No patient died intra-or peri-operation.No serious postoperative complained during the peroperative period.The number lymph nodes removed was 225.No patient died until October 2011.Conclusion:Video-assisted thoracoscopic and laproscopic esophagectomy is technically feasible and safe,with reduced surgical trauma,bleeding,incisional pain and decreased postoperative lung complications during the preoperative period.It has the potential to replace open esophagectomy in selected patients.
