烧伤严重程度分类的年龄分组

目的探讨烧伤严重程度分类的年龄分组。方法将26036例烧伤患者的病历资料录入数据库,用SPSS统计软件统计。先将对病死率影响较大、又与烧伤面积大小无关的吸入性损伤、伤前疾病和合并伤病例去除,将年龄分成12组。在0.5%病死率时求出各组的例数、最大烧伤面积、平均烧伤总面积、平均Ⅲ度面积。根据统计学差异、平均烧伤面积和临床再将12组合并成3组。结果用方差分析和卡方检验其显著性,并用LA50和生存分析验证其合理性。结果年龄被分为≤2岁组、≤55岁组、>55岁三组,≤2岁组和>55岁组与≤55组之间有非常显著性差异(P<0.01)。结论≤2岁、≤55岁、>55岁可作为烧伤严重程度年龄分组界限。

Discussion on age-grouping in burn severity classification

Objective To discuss the possibility of age-grouping in classifying the severity of burnt patients. Methods 26 036 records of burnt patients in author's database were analyzed with SPSS (version 11.0). After excluding the patients associated with complications,which were independent of burn surface areas but made a great impact on the mortality rate of patients,such as inhalation injuries,preexist diseases,and combined injuries,other patients were divided into12 groups according their different ages. The numbers of patients,the maximal total burn surface areas (TBSA),the average TBSA,and the average third degree burn surface areas(BSA) of all groups were calculated and analyzed at the three mortality points of 0.5% respectively. The former 12 groups were combined to form 3 groups according to their statistical differences,average TBSA and clinical features. The different significances were tested by analysis of variance and chi-square criterion,and the rationality of the age grouping was supported by the LA50 and survival analysis. Results The patients were divided into three age groups: ≤2 years,≤55 years and >55 years group. There was a significant difference between the ≤2years group and the ≤55years group,and the>55years group and the ≤55 years group(P<0.001).Conclusion The above-mentioned three age-grouping (≤2 years,≤55 years and >55 years group)can be taken into account as an important foundation in the burn severity classification.
