依托咪酯与丙泊酚联用于老年患者无痛胃肠镜检查的效果比较

目的比较3种静脉麻醉方法在老年患者无痛胃肠镜检查的临床效果。方法胃肠镜检查术的老年患者180例，随机分为3组：A组给予丙泊酚1．5～2mg／kg；B组给予依托咪酯0．2～O．25mg／kg；C组先静脉注射丙泊酚30~40mg，然后再给依托咪酯10～15mg。观察患者在检查时的血液动力学指标、脉搏血氧饱和度（SpOz）、苏醒时间及不良反应。结果两组患者的苏醒时间及检查过程中的Sp02差异均无统计学意义（P〉0．05）。与注药前比较，注药后A组患者血压下降较明显，C组注射痛及肌颤发生率较低，差异有统计学意义（P〈0．05）。结论丙泊酚和依托咪酯联合应用效果确切，不良反应少，更适于老年患者无痛胃肠镜检查。

Comparison of etomidate versus propofoi or combination in elderly patients with painless gastrointestinal endoscopy

Objective To compare the effects of etomidate and propofol in three different intra- venous anesthesia methods in the elderly patients undergoing painless gastrointestinal endoscopy. Methods One hundred and eighty elderly patients undergoing painless gastrointestinal endoscopy were randomly divided into three groups. In group A the patients received propofol, in group B re- ceived etomidate, and in group C received propofol and etomidate. The recovery time, adverse reac- tions and BP, SBP, DBP, HR, SpO2 were observed. Results There was no significant difference in the recovery time and SpOz among the three groups （P〉0.05）. Compared with preanesthesia, BP was significantly decreased in group A. The incidence rates of injection pain and muscle twitch in group C were lower than those in the other two groups（P〈0.05）. Conclusion Propofol combined with etomidate can provide effective pain relief during injection with fewer adverse reactions in the elderly patients with painless gastrointestinal endoscopy.
