米索前列醇不同给药途径联合异丙酚在无痛人工流产术中的应用

目的 观察无痛负压吸宫术前应用米索前列醇的宫颈软化作用及药物不良反应的发生率。方法 选取无阴道分娩史、初次行无痛人工流产术患者180例,按就诊顺序随机分成3组,每组60例,A组单用异丙酚行无痛负压吸宫术,B组阴道后穹窿放置米索前列醇400μg,3小时后行异丙酚无痛负压吸宫术,C组口服米索前列醇400μg,3小时后行异丙酚无痛负压吸宫术。观察患者无痛效果、宫颈松弛情况、术中出血量、手术时间及药物不良反应(恶心、呕吐、腹痛、腹泻及术前阴道流血)。结果 宫颈松弛作用、麻醉效果B组和C组明显优于A组,且B组优于C组(P<0.05);B组药物不良反应、恶心、腹泻发生率明显低于C组(P<0.05)。结论 在无痛负压吸宫术前使用米索前列醇可有效松弛宫颈,提高异丙酚麻醉满意度。

Clinical application of Misoprostol with different administration routs complicating Propofol for painless induced abortion

Objective To study clinical effect and drug adverse reaction of Misoprostol with different administration to soften the cervix and facilitate cervix dilatation in surgical abortion complicating Propofol total intravenous anaesthesia.Methods Totally l80 women opting for voluntary surgical abortion were randomly allocated to three groups:Group A with 60 cases that were taken painless uterine aspiration by negative pressure complicating Propofol,Group B with 60 cases that received Misoprostol(400 ug)being placed in posterior fornix and after 3 hours were taken painless uterine aspiration by negative pressure,Group C with 60 cases that were taken oral Misoprostol(400 ug)and after 3 hours were taken painless uterine aspiration by negative pressure.The data of cervical relax and dilatation,the volume of bleeding,the time of the operation,the side effect(nausea,vomit,abdominal pain,diarrhea,vaginal bleeding)during the operation were observed.Results The difference of cervical dilatation and anesthetic effect was significant between B or C group and the control group(P<0.05).The incidence of adverse reaction,nauseated,diarrhea in Group B was obviously lower than that in Group C(P<0.05).Conclusion Misoprostol administrated intravaginal is a routine application on the effect of cervical dilatation and anesthetic before induced abortion.
