肝癌患者的血浆纤维蛋白原水平变化及意义

目的探讨具不同肝硬化背景的肝癌患者血浆中纤维蛋白原（Fibrinogen,FBG）水平差异及其临床意义。方法采用全自动血凝分析仪检测48例正常人、104例肝硬化患者、243例肝癌合并肝硬化患者、61例肝癌无肝硬化患者的FBG,分析四组间血浆FBG水平的差异;并分析FBG水平与是否合并肝硬化、肝功能分级、原发灶大小、肝内血管癌栓形成情况、肝癌分期间的相关性。结果肝硬化组、正常对照组、肝癌合并肝硬化组、肝癌无肝硬化组的血浆FBG水平依次升高;肝功能分级越差、原发灶越大、肝内血管癌栓越多、肝癌分期越晚的患者血浆FBG水平越高,差异均具有统计学意义（P〈0.05）。结论肝癌患者具有高纤维蛋白原血症的特点,肝癌有无肝硬化背景与患者血浆FBG水平相关,血浆FBG水平可能成为肝癌发生发展、浸润转移的预测性指标之一。

Plasma Fibrinogen Levels in Hepatocellular Carcinoma and its Clinical Significance

Objective To evaluate the plasma Fibrinogen（FBG） level of patients with hepatocellular carcinoma（HCC） with or without liver cirrhosis and its clinical significance.Methods The subjects were divided into four groups：48 normal adults（A）,104 patients with liver cirrhosis（B）,243 patients with HCC and liver cirrhosis（C）,146 patients with HCC without liver cirrhosis（D）.Plasma FBG levels were measured by fully automated coagulation analyzer.The differences of plasma FBG level of four groups and the relationship between plasma FBG level and some clinicopathological parameters were analyzed.Results Plasma FBG level in group B,group A,group C and group D raised successively.Plasma FBG level was higher in patients who had worse Child-pugh grade,larger tumor,more satellite lesions,more intrahepatic vascular carcinous-thrombosis and higher clinical stage.Conclusion Patients with HCC showed hyperfibrinogenemia characteristic.HCC with or without liver cirrhosis affected plasma FBG levels.Plasma FBG level may be a predictor in progression and metastasis of HCC.
