金属烤瓷冠桥修复115例疗效观察

目的评价金属烤瓷冠桥修复的临床效果。方法对口腔科2007年1月-2009年12月期间115例烤瓷冠桥的临床资料进行了回顾分析。结果经636个月随访观察,复查的115例患者中,成功105例,占91.3%;失败10例,占8.7%。在修复失败的病例中烤瓷固定桥失败4例,烤瓷桩核冠失败4例,烤瓷全冠失败2例。在失败的病例中瓷崩裂2例,桩核冠脱落2例,基牙折断1例,基牙并发牙髓炎1例,龈缘红肿1例,食物嵌塞1例,龈缘变色1例,色泽不协调1例。分析失败因素主要有崩瓷,修复体松动脱落,基牙病变,牙龈明显红肿,食物嵌塞,美观因素。结论金属烤瓷冠桥能很好地恢复患者的咀嚼功能,在使用烤瓷熔附金属冠桥修复牙齿缺损及缺失时,应正确选择适应症,合理进行设计,严格规范临床操作程序,才能进一步可提高金属烤瓷冠桥修复的成功率。

The Clinical Observation of 115 Porcelain Fused to Metal Prostheses

Objective To evaluate the clinical effect of porcelain fused to metal prostheses.Methods From January 2007 to December 2009,115 cases of porcelain fused to metal prostheses were analyzed retrospectively.Results The review of 115 patients,after 6 to 36 months follow-up study,105 cases were successful,accounting for 91.3%,10 cases of failure,accounting for 8.7%.In these failed cases,4 for bridge,4 for porcelain crown,2 for porcelain crown.In these failed cases,2 for fracture,2 for crown lost,1 for abutment fracture,1 for pulpitis,1 for gingival margin inflamed,7 for food impaction,1 for discoloration of the gingival margin,1 for inconsistent color.The main factors of failure are fracture,crown lost,abutment disease,gingival inflamed,food impaction and aesthetic reason.Conclusion Porcelain fused to metal prostheses is an internationally popular repair method at present and it can recover masticator function well.In the use of porcelain fused to metal crown and bridge repair defects and missing teeth,they should correct choice of indications rational design,strictly regulate the clinical operating procedures in order to improve the success rate
