无痛胃镜在高原原发性高血压患者的实用性探讨

目的探讨无痛胃镜在高原原发性高血压患者胃镜检查中的实用性。方法将60例行胃镜检查的高原高血压患者随机分为无痛组和常规组,各30例。无痛组先予曲马多静脉推注,再予丙泊酚静脉推注,待睫毛反射消失后,即开始进镜;常规组仅行咽部麻醉后,即开始进镜。观察两组患者检查前、检查中、检查后收缩压（SBP）、舒张压（DBP）、心率及脉搏血氧饱和度（SpO2）的变化,无痛组还记录清醒时间。结果由于吸氧两组SpO2无明显变化（P〉0.05）;常规组检查中较检查前SBP、DBP升高、心率加快（P〈0.05）;无痛组检查中较检查前SBP、DBP、心率下降（P〈0.05）,检查结束后基本恢复正常。结论丙泊酚和曲马多联合应用于高原高血压患者无痛胃镜检查是可行、安全的,值得临床进一步推广。

Practical Applicability of Propofol Combined with Tramadol on Analgesia Gastroscope in Patients with Essential Hypertension at High Altitude

Objective To investigate the practical applicability of propofol combined with tramadol on analgesia gastroscope in patients with essential hypertension at high altitude.Methods Sixty cases of plateau hypertensive patients were randomly divided into painless group（n=30） and general group（n=30）.The painless group received tramadol intravenous injection,then propofol intravenous injection;the gastroscopy began when the eyelashes reflection disappeared;whereas the general group only received the pharyngeal narcotic.The systolic blood pressure（SBP）,diastolic pressure（DBP）,heart rate（HR） and pulse oximetry（SpO2） were observed in,before and after the operation.And the recovery time of patients in the painless group was also recorded.Results There was no obvious difference in SpO2 between the two groups（P〉0.05）.In the general group,SBP,HR and DBP of patients increased as compared the values before the operation（P〈0.05）.In the painless group,SBP,HR and DBP decreased in the operation（P〈0.05） and recovered after the operation.Conclusion Propofol combined with tramadol is feasible and secure in painless gastroscopy in patients with essential hypertension at high altitude.It is worth being popularized in clinic
